JOB DESCRIPTION Main Duties and Responsibilities Communications To work with the Infant Parent Service & Perinatal Mental Health Team Parenting Team in the promotion of access to services and provision of information and involvement of multi-agency colleagues. Work in a person centred way and always consider the impact of poor Mental Health, considering the voice of the child, in the context of the family and community functioning. Work with parents collaboratively, using highly developed communication skills and empathy, considering complex and sensitive information in the context of the perinatal MH services which are trauma informed. Contribute to multiagency meetings and planning, and support services in the completion of high quality assessments & provision of care Prepare, write and record assessments, case notes, contacts and written reports accurately and promptly in accordance with departmental and national protocol and guidance as relevant. Undertaking monthly supervision of any cases with the lead specialist mental health midwife. To foster good working relationships across multi-agency teams in a variety of settings. Responsibility for Patient Care The post holder will take responsibility for the processing of MH referrals from Midwifery colleagues. Providing information to service users at the request of the specialist mental health midwives which is individualised in conjunction with the multi-agency team. The post holder will offer women and their families support, non- clinical advice and information on a range of issues. Providing assessment and signposting with difficult and highly expressed emotions in the families and responding effectively to safeguarding needs The post holder will with direction from the Specialist MH midwives, work with women & families to establish relationships, to explore solutions and plan interventions based on the assessment where family functioning and participation in engagement is difficult or complex Provide 1:1 support to women as requested by the specialist mental health midwives to offer role appropriate management of mental health symptoms. This may be via phone/ video/ face to face at home or on hospital settings. Providing practical support for women and their families including advice of Safe Sleeping and the importance of the first 1001 days. Provide women and their families with information on the parent and infant relationship to optimise parent and infant mental health and promote bonding with their babies. Being alert to parents who are a high risk of harm to themselves, others, or their babies and escalating any concerns. To maintain accurate and contemporaneous paper and electronic records in accordance with Trust policy and follow data security, confidentiality and information sharing policies. To advise his/her manager and/or Specialist Safeguarding Nurse of concerns or suspected child abuse and adhere to the procedures defined in Stockport Safeguarding Children Guidelines prepared by the Stockport Local Safeguarding Children Board Planning and organising Support with booking new and follow up appointments, including onward referrals to other services Receive and screen incoming postal and electronic mail, identifying appropriate action or referral and initiating follow up action where necessary. Attend meetings and take minutes with the specialist mental health midwives. Have the ability to engage in and contribute to the on-going Care Plan Review Meetings, and Child In need and Case Conferences. Work in a variety of settings including family homes. To demonstrate responsibility for ongoing professional development. Undertake relevant administrative duties and maintain appropriate records in accordance with the role and following information governance. Organise and plan own day to day workload or activities in order to meet the demands of the job role. Responsibilities for Physical and / or Financial Resources Be responsible for the organisation of clinical records and referrals, including appropriate safe handling of all documentation. Ensure that all office stock and stationary supplies are maintained. To be familiar with the use and safety of equipment. Responsibility for Policy and Service Development and Implementation Participate in and contribute to the effective introduction of new systems leading to the improvement and development of patient services. Ensure complaints/ compliments/ comments regarding the service are reported appropriately at the first available opportunity. Attend relevant training sessions and meetings. Ensure display boards throughout the maternity unit provide up to date relevant information related to perinatal mental health. To follow all Departmental and Trust policies and procedures. To contribute to service improvement initiatives and promote high quality care for patients at all times. To comment on protocols and procedures relating to area of clinical care. Responsibilities for Human Resources and Leadership To escalate identified areas of concern from families to the appropriate manager. To attend supervision and training re safeguarding children. Support new or less experienced work colleagues to adapt to the workplace. To attend all education and training needed to maintain effective practice. To facilitate professional development of self and students. To participate in the appraisal process for self, ensuring that objectives and development plans are in place and updated on an annual basis. To create, maintain and enhance effective working relationships with immediate and multi-agency colleagues. Responsibilities for Teaching and Training Participate in training and development activities that are relevant to the job role. Responsibilities for data and information resources . The post holder will work with management and the IT department to ensure systems are effective and fit for purpose The post holder will introduce and champion new electronic ways of working that promote integration and safer care. The post holder will ensure effective use of I.T. initiatives within perinatal MH provision and raise issues of concern where they affect service provision. To collect and use local data in order to inform local delivery and planning of interventions to improve outcomes for women, families & children. Collect, collate and present data utilising a defined methodology, in a timely way in order to inform evaluation, performance and planning Research, Development and Audit Capture data and monitor the experiences of women within the midwifery mental health team. To maintain up to date knowledge of research, NICE guidance etc. to ensure practice is current and evidence based. To contribute to governance and quality assurance, reducing risk to children, families and staff. Physical Skills and Effort Car driver Perform other reasonable tasks that may be necessary for the efficient running of the midwifery mental health team. The range of duties and responsibilities outlines above are indicative only and are intended to give a broad idea of the range and type of duties that will be allocated. They are subject to modification in the light of changing service and demands and the development requirements of the post holder. The Trust is committed to safeguarding and promoting the welfare of children, young people and vulnerable adults and expects all staff and volunteers to share this commitment and promote safeguarding by implementing the Trusts policies and procedures, acting promptly on concerns, communicating effectively and sharing information appropriately. If successful this vacancy will require an enhanced DBS check at a cost of £41.90. This cost will be deducted from your first Trust salary In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £13.
