Job description


We are looking to recruit a Nurse who is highly innovative, enthusiastic, with passion for delivering high quality patient centred care, in a forward thinking and MSNAP accredited memory service.

The role requires the successful candidate to complete memory assessments, balancing risk, and dealing with patients with complex needs within a friendly and supportive team.

The post holder must have proven clinical skills along with excellent communication skills, good knowledge and understanding of dementia and the needs of individuals diagnosed with this condition. For this position it is essential that you have post qualification experience and are able to demonstrate a high degree of clinical skills and knowledge. You will be based at Broad Street Centre in Dagenham and will be supported and supervised by the memory service Clinical Lead Nurse.

You will be part of the well-established multi-disciplinary team comprising of nurses, psychologist, occupational therapists and psychiatrists.
• Providing assessments and supporting diagnosis for people with memory problems
• Comprehensive assessments in the Service User’s own home.
• Ensuring assessments are comprehensive and informative to aid diagnosis (Using agreed assessment tools i.e MoCA HADS etc)
• Ensuring that all referrals are seen in a timely manner
• Mentoring junior staff members.
• To be practice assessor/supervisor for student nurses
• Advocating for the Service User
• Support the Memory Service to maintain its MSNAP (Memory Services National Accreditation Programme) accreditation.
• To be trained as MSNAP Peer Reviewer (Training provided by Royal College of Psychiatry)
• Development and facilitation of psychotherapeutic interventions within the Memory Service.

Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

COVID-19 Vaccination
We continue to encourage all staff to ensure that they have been double vaccinated and received their booster. We recognise that taking the vaccine provides the best defence against COVID 19 for our patients, our staff and their families.

Probationary Period
This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

High Cost Area Supplement – London
This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,313 and a maximum payment of £5,436 per annum pro rata).
1. To be responsible for organising and prioritising own and others workload in the day to day allocation of work.
2. To have organisational knowledge relating to Trust protocols and procedures and adhere to them, particularly administration of medicine and moving and handling.
3. To be responsible for providing accurate records of information required by the Trust for audit purposes.
4. To ensure effective risk management at team level by accident/incident reporting, assessing and controlling risk and ensuring residual risks are added to the Trust register.
1. To work effectively with GPs to ensure early referral and diagnosis
2. To work jointly and collaboratively with NELFT colleagues in order to enhance and develop services to service users and their families.
3. To ensure communication channels are consistent and understandable and that information provided is relevant and informed.

Leadership
1. To participate in the development of an effective team and the development of productive working relationships throughout the Trust.
2. To participate in the induction of new members of staff as a mentor and preceptor for nurses.
3. To assist in the recruitment of staff.
4. To participate and review staff performance.

Clinical Skills
1. To act as an autonomous, registered practitioner who is legally and professionally accountable for own unsupervised actions guided by the professional code of conduct and Trust guidelines and protocols.
2. The post holder will have Current Effective Status on the Nursing and Midwifery Council (NMC) – Registered Nurse (RN12).
3. To be responsible, and accountable, for service delivery to clients/patients.
4. To be able to assess and develop plans of care to meet the complex and diverse needs of patients with a variety of conditions. This includes chronic, acute and palliative care within own competencies, recognising own limitations and seeking advice when necessary. This will include continuously evaluating and acting on outcomes.
5. To be able to initiate referrals to other health professional specialist services and agencies.
6. To provide patients and relatives with information and education thus ensuring they have meaningful choices that promote dignity, independence and quality of life.
7. Further professional knowledge will have been gained through accredited courses workshops study and in house training programmes.
8. To ensure practice is supported by research, evidence based practice, literature and peer review.
9. To be responsible and accountable for service delivery to clients.
10. To provide and facilitate post diagnostic support to patients and their carers.

Computer/Administration
1. To be computer literate and encourage implementation of the Trust’s IM&T Strategy.
2. To ensure accurate recording of actions, and updating patient’s records, maintaining confidentiality at all times.
3. To take part, and assist, in the planning and administration relating to day to day running of the caseload.

Communication

To have a wide range of knowledge in approaches to communicating and managing patient care.
1. To be able to effectively communicate with colleagues, peers, senior managers and clinical leads within the Trust.
2. To be able to communicate complex patient related information facilitating positive outcomes and ensuring collaborative working.
3. Participate in the review and development of clinical policies and identifies improvements to service provision.

Training
1. To act as practice assessor/supervisor to students, providing effective education, facilitating their development, and promoting high standards of nursing care.
2. Ensure students are actively supported to enable them to achieve their learning needs.
3. To ensure own continued professional development and support a culture of lifelong learning in self and others.
4. To undertake, and assist, in the planning of own mandatory training and workshops.
5. To undertake a regular appraisal, developing a personal development plan that includes clinical competencies reflecting the health needs of the local population and relates to Trust strategy.
6. To support new staff and their integration within the team.
7. To actively engage in training as part of the role including changes to professional development and implementation of new policies and guidelines.
