Job description
In your new role you will: Be responsible for supporting service users with a learning disability and health need in the community intensively working shifts over a 7-day rota Carry out assessment of individual needs, make recommendations of how to meet those needs, support the implementation of these recommendations and monitor, evaluate and review the input Work in close liaison with Team Manager, Nurse Team Leader, and Clinical Leads in delivering effective evidence-based practice to adults with Learning Disabilities requiring mental health services Take a lead in and share the responsibility for identifying and developing opportunities for Service Users with learning disabilities to be enabled to comment on local and Trust wide service operational and development.
