Job description

Main Duties: Through specialist knowledge and experience, guide health care staff within RSCH to better understand the complex presentations associated with PWLD and to anticipate possible problems and further develop individualized care management plans for PWLD Collaborate with other health care professionals/relevant agencies within RSCH to promote planned and coordinated care Demonstrate a highly developed interpersonal approach with team members, patients, staff teams, families and supporting agencies where there are sensitive and contentious situations Active promotion of the liaison nurse role in RSCH, ensuring information is accessible to PWLD and their carers Assist PWLD, staff and carers pre-admission, at admission, during their hospital stay and at discharge to facilitate excellent care Facilitate an environment in which ethical issues are recognized, debated and acted upon. To meet regularly with identified Lead in RSCH. To actively promote equality and diversity issues for people with learning disabilities in RSCH To promote the use of patient held health action plans when clients access RSCH Adhere to the guidelines laid down by the NMC Code of Professional Conduct and standards on Records and Record Keepin To be involved in assessments of capacity to consent and multi- disciplinary best interest meetings for PWL To understand the national context which informs learning disability practice as it relates to the acute hospital setting e.g. findings from the learning from lives and deaths (LeDeR) reports and implications for practice in preventing avoidable and premature deaths of PWL To be able to provide and receive highly complex information, in difficult and challenging situations, and be able to address and resolve barriers to understanding and acceptance of clinical and behavioural interventions where there may be different options. This will involve sharing relevant information to patients, staff teams, families and other supporting agencies
