A Vacancy at Rotherham Doncaster and South Humber NHS Foundation Trust.

We are pleased to be able to offer a Clinical Lead Mental Health Occupational Therapist post in Rotherham within Primary Care Integrated Mental Health. You will be joining an established team of Mental Health Practitioners within Swallownest Court Rotherham and working with locality Occupational Therapists within South and North Community Mental Health Team. We are dedicated about ensuring that our patients see the right person in the right place, first time and receive quality intervention. You will be supported by a wider team of clinicians.

The ability to take a lead role in completing autonomous assessments, triage and treatment of acute and chronic presentations for patients with mental health concerns attending primary care.

Maximize/maintain independence in preferred home environment and proactively resolve health and social issues at an early stage, minimizing crisis situations that result in hospital presentation/admission.

To help patients identify their needs and make shared decisions about management using current evidenced based practice.

Liaise with Primary Care colleagues to ensure that all the patients needs are addressed increase sustainable discharge and support in community.

Release professional capacity by focusing intervention on enabling people to maximize their potential, prevent ill health and dependency, using skills of full MDT.

Work collaboratively within the developing Locality Teams.
• Support Primary Care colleagues development in the best treatment and onward management strategies by attending meetings, delivering educational sessions and supporting PCIMH Practitioners.
