Job description
Communication and working relationships Liaise with the system provider to ensure smooth processing of the database. Support the clinical and administrative team working towards systems and processes which allow full realisation of the tracking benefits of the database. To communicate effectively in writing and verbally on a daily basis. Support the nursing team with taking messages, logging calls and actions on Infoflex. Liaising with the Patient Access Centre to book outpatient clinics and appointments. To work closely with all grades of clinical and administrative staff, along with other employees within the Trust including: Clinical Leads Medical staff Nursing staff Secretarial, administrative, and clerical staff Pharmacy services Diagnostic services Biologics coordinators Planning and organisation Provide training for other members of the Team in the use of the database. Able to receive, handle, analyse and resolve data queries promptly. Direct unresolved queries to appropriate team member. To plan and organise own workload efficiently and to highlight any concerns to the Specialist Nurses. To ensure all results of tests and investigations are tracked effectively, shown to the relevant consultant promptly and to undertake appropriate action as instructed. To sensitively deal with patient/carer telephone calls and enquiries in an efficient, sensitive and confidential manner, exercising judgement to analyse and resolve any problems and referring on to the clinician as appropriate. All clinic outcomes must be recorded accurately and complete or amend any that have not been done where appropriate. Preparing notes and taking and transcribing formal minutes for MDT meetings and other meetings as and when required. To open and act on correspondence received and bring to the attention of the specialist nurses along with the medical notes. Performing ad-hoc analyses of data and reporting when requested and sharing with the lead clinicians. To undertake a variety of office duties such as filing, opening and distributing post, photocopying, moving and lifting of patient notes to ensure the efficient and effective support to the department. To produce and update Standard Operating Procedures (SOP) for all aspects of the role. Collect, collate, evaluate, and report information as required. Maintain accurate patient records. Effectively work within the service to ensure that it is responsive and flexible. Manage own time with conflicting priorities to ensure that the needs of patients, workforce and service are met. Responsibility and accountability Be responsible for complex data collection and maintain quality and content of data held on the database system. Coordinate (under supervision of consultant), protocol generated assessments and questionnaires to support clinical audit reports and patient surveys. To ensure that all clinical and non-clinical correspondence is typed within local targets, using the Infoflex system. (Audio and copy typing). Ensure appropriate follow up is actioned i.e. outpatient appointments, investigations, patients added to waiting lists, referrals to other departments etc. To ensure that all results/reports/correspondence are filed in the hospital case notes in accordance with Health Records standards. To ensure that private, overseas and Scottish/Welsh and Northern Irish patients for out/in-patients are identified and documented appropriately. Any other appropriate duties/tasks as requested by management. Maintain own personal and professional development, undertake all mandatory training required for trust compliance and for the development of the role. Policy and service responsibility Assist in patient satisfaction surveys and clinical audits. To comply with the Trusts Patient Access Policy and Standard Operating Procedures. Demonstrate self-directed learning, actively seeking role development opportunities to enhance practice, knowledge, and role progression. To identify areas for service improvement and support implementation. Responsibility for finance, equipment and other resources Ensure flexible working and working to rostering KPI guidelines. Be aware of and assist budget holder for efficient and effective usage of the available resources, working within budget. Ensure the effective and efficient use of physical & financial resources. Responsibility for supervision, leadership and management No supervisory or line management responsibilities. Information technology and administrative duties Undertake accurate and timely data entry using the database and/or paper-based systems, ensuring attention to detail and that systems are up to date. Concentration required for data input accurately for prolonged periods of time Assist in patient satisfaction surveys and clinical audits. Enable access to the database for new users. Assist in setting up new admin processes to support monitoring of patients OP attendances, blood results and prescriptions. Collating prescriptions and following standard processes to allow smooth delivery of medication to patients. Assist team members in preparation of accurate reports and presentations. Undertake any other tasks required that are in line with the banding of this post. Ensure a clear understanding of data quality, data protection and patient confidently surrounding the capture and reporting on the department systems. Monitoring Specialist nurse clinics and ensuring all patients receive their appointments within the correct timeframe and that all clinics are utilised appropriately. To enter, monitor and update all aspects of the Referral to Treatment (RTT) pathway events accurately and ensure Inter Provider Transfers (IPT) are completed and updated. Review statuses are to be updated at every patient interaction. To use the relevant IT systems to register patients, respond to enquiries, make appointments, transfer medical records, input clinical letters, and care planning summaries, as required. To cover for members of the secretarial team when required. Actively look for ways to develop the service. Provision of information sharing where appropriate, on a need-to-know basis. Provide trust statistics to Public Health commissioners, e.g. Integrated Care Board, NHS England, Public Health England.
