Job description
The following are the core responsibilities of the Reception/Care Navigation Team Leader. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels: Overseeing the reception services of the practice, ensuring staff achieve their primary responsibilities Line managing all reception staff, supporting staff development, providing guidance and direction, ensuring staff are up to date with mandatory training Completing staff appraisals as required Identify and deliver team training where required Compiling reception staff rota ensuring sufficient cover is provided for periods of leave and other staff absences Reviewing and updating all reception procedures as required Support the management team in the compilation of practice reports and the practice development plan Develop, implement and embed efficient reception processes and procedures to adhere to extant legislation Maintain an effective appointment system for all services Act as a focal point for communication, advising reception and administrative staff on methods of communication and best practice Update appointment system to reflect leave and other approved absences Provide initial guidance and advice to patients who wish to complain Act as building fire Marshall Administer the Holiday Manager System for members of the Reception Team Carry out system searches as requested Monitor and maintain the reception area Monitor and maintain the patient checking in machine, uploading room plans, new messages as needed Supervise, monitor and maintain the telephone system (including call volume) Monitor the quality of call handling and action any improvements needed Be aware of new technologies that will improve service Be aware of service changes that will effect Reception and plan accordingly Complete opening and closing procedures in accordance with the duty rota Monitor and maintain the reception area and noticeboards Maintain room plans making the best use of space and ensuring all clinicians have appropriate rooms Maintain the EMIS diaries for clinical roles Lead on patient access to their records, updating records as needed and monitoring age groups Lead and monitor identifying out of area patients and following through the correct process Supervise the registration of new patients Supervise the pathology workflow Oversee the organisation of the home visits making best use of the appropriate services Attend management meetings Hold regular team meetings Care Navigation Lead on and embed the care navigation process Process and effectively signpost patients to the appropriate healthcare professional, depending on the presenting condition Answer incoming phone calls, transferring calls or dealing with the callers requests appropriately Process patient requests for appointments Process repeat prescription requests Initiate contact with and respond to requests from patients, team members and external agencies Data entry of new and temporary registrations and relevant patient information as required Input data into the patients healthcare records as necessary Direct requests for information, e.g., SAR, insurance/solicitors letters and DVLA forms, to the appropriate team Manage all queries as necessary in an efficient manner Carry out system searches as requested Action incoming patient facing messages when necessary Order and monitor secure stationery supplies
