Job description
The post will involve working Rotherham Home Treatment team, working with people who at risk of admission to an Acute mental health ward. The team also support people on an Early Discharge pathway from the Acute wards. The team provide intensive support in the home environment. It will involve working with people who have may have complex needs, involving mental health needs, social care and psychological needs.
You will be part of a multi-disciplinary team that includes Mental Health nurses, Mental health social worker, and Psychiatrists.
You will work as a member of the team providing high quality care- working. You will be involved in delivering the care plans agreed in the MDT, as well as supporting people to access their local communities.
You will work jointly with service users and their family carers, including paid carer services. Offering family support is a key role.
You will develop close working links with partner agencies, promoting joint working in a co-ordinated manner.
You may be required to work on your own for periods of time without direct supervision, on visits that have been risk assessed, and will have access to supervision with clinicians and the Team Manager.
You will be required to be involved in the collection and collation of patient and carer feedback.
You will be required to work flexibly over 7 days per week between the hours of 9am and 7pm.
The role covers the geographical area of Rotherham.
• You will be required to support individuals via telephone, within the team base and patients own homes.
• Enabling patients to engage effectively with identified plans of care.
• Positively promoting independent living in the community.
• Supporting patients with recognising ways of coping and building in coping strategies.
• Encouraging access to local support services
• Supporting patients to establish and maintain effective relationships.
• Provide practical support for patients and carers in managing independence.
• Providing carers assessments and associated work.
• Recognising safeguarding issues and reporting accordingly
• Participating in the MDT and providing updates
• Recording information in the electronic care record.
• Completing outcome tools with patients such as REQol 10
• Supporting patients to attend appointments.
• Signposting to appropriate services
• Assisting in physical health clinic (promoting smoking cessation and healthy eating)
• Providing support accessing benefit and Housing advice by signposting appropriately to Social care
• Connecting people within the community.
• Promoting independence and choice
