About the role
- Support Care Advisors to review and update clinical care plans identifying opportunities for best practices
- Provide clinical supervision and support within the scope of practice as per AHPRA registration.
- Review and follow up on clinical issues, identified risks, hazards, incidents
- Implementation and monitoring of clinical compliance requirements for the Aged Care Quality and Safety Standards.
- Maintain and enhance professional relationships with customers and their families, brokered service providers, internal team and stakeholder

Desired Skills and Experience
About you:
- Tertiary qualifications in Nursing or Allied Health discipline
- Experience in aged care client assessment and care advisory in Home Care Package
- Experience with supporting Case Management in particular with clinicaloversight
- Demonstrated understanding of Home Care Standards, best practice clinical guidelines & community quality reporting process
- Demonstrated understanding of Aged Care Quality Standards and Home Care Program guidelines
- Strong attention to detail
- Excellent verbal and written communication skills
- Able to work independently and as part of a multidisciplinary team
- Need to have a driver’s licence and working rights in Australia
