Job description
We are looking for a Transformation Project Manager to join our supportive and collaborative Transformation Team on a one-year fixed term or secondment basis. The Transformation Team supports the delivery of key strategic transformation across Hertfordshire Community Trust and this role will have a particular focus on working with our Children’s and Young People (CYP) Service to improve the way in which we provide care and support for neurodiverse children. The post holder will work closely with CYP Specialist Services leadership, clinicians, and teams, as well as engaging with system partners and service users or their families.
The post holder will use their experience and demonstrable project management skills to ensure that projects are well planned, efficiently implemented, and best practice project methodology is used to
ensure the project is a success and the identified benefits are realised. They will work in collaboration with senior colleagues, clinicians, technical subject matter experts, frontline staff and a range of colleagues from within the Trust and from partner organisations and commissioners.
All members of our Transformation Team are encouraged and supported to use their own creativity, ideas and skills to work in partnership with colleagues to address challenges, identify opportunities for improvement and help HCT to be an organization that seeks to continuously improve what we do.
The post holder will manage the successful delivery of a range of projects and work areas across Hertfordshire Community NHS Trust to support organisational improvement and transformation. The post holder will work across the Trust providing support for projects that are both internal and external facing, will analyse requirements, support the governance and work hand-in-hand with colleagues from across the organisation to develop and enable new and improved ways of working that supports our delivery of safe and high-quality patient care.

1. Manage all project resources and relevant project documentation ranging from project start-up, progress reports, risk and issue logs and project close-down reports
2. Maintain and manage project plans for all allocated projects.
3. Communicate project progress, including to any relevant program boards, celebrating progress and effectively escalating challenges to seek support and solutions
4. Ensure the project remains on track to deliver outcomes in line with time, cost and quality constraints.
5. Establish and manage relevant project meetings to ensure the proper functioning of the project
6. Use own creativity, ingenuity, ideas, and analysis of relevant data to support colleagues to identify best practice and possible improvements
7. Actively contribute as a vital member of the Transformation Team and wider Strategy Directorate, enthusiastically supporting colleagues and attending and contributing to team and directorate meetings
8. Supporting our desire to continuously improve, members of the Transformation Team are supported to develop, learn and improve. All team members then have a key role in bringing any learning back to the team to support the development of others
