Job description
Due to a retirement we are looking for an enthusiastic and highly motivated Clinical Pharmacist to join our friendly and patient-centred GP practice. We are a team of 4 Partner GPs, 2 Advanced Nurse Practitioners, 6 nursing team members, prescription management team, full administration team comprising of medical secretaries, Performance Manager, Patient Services Manager, reception team, General Practice Assistant and Health Coaches.
The post holder will support, promote and implement high quality, evidence-based, cost effective and safe use of medicines. This includes patient medication reviews, helping the practice achieve prescribing targets and supporting the implementation of national and local guidance in order to improve patient care. Main duties of the job
- Provide sessional clinical support as an independent pharmacist prescriber, seeing patients and leading on medicines optimisation.
- Support the surgery to improve medication reviews, medicines optimisation and formulary adherence.
- Actively participate in regular weekly team meetings, providing advice on appropriate medications and de-prescribing where appropriate, as well as ad-hoc support to the PCN care home team
- Support the practice clinical team to authorise patient medication requests
- Support and provide day to day management of the prescription team in carrying out their duties
- Support our plans to reduce medicines waste in primary care
- Interpret and provide complex medicines information to patients, carers and healthcare professionals
- To provide information, advice and guidance on the safe, rational, effective and economical use of medicines
- Undertake clinical audit and other outcome measures to support patient care and safety
- Create and implement medicine related guidelines, policies and SOPs as appropriate to the different service areas
- Provide and receive complex, sensitive information and communication relating to pharmaceutical care of patients
- Provide prescribing service within sphere of competency to patients in primary care, as pharmacist independent prescriber
- To develop, advise, agree and implement action plans at a practice level, monitor the effects of changes made and ensure that any benefits are maintained in the longer term
Overview of our organisation
We use EMIS Web as our clinical system supported by a range of other computer-based tools and have been at the vanguard of some of the developments in primary care in recent years.
We see teamwork and quality of life as important attributes of the practice, with daily pre-clinic huddles and morning coffee each day as well as more formal weekly practice and clinical meetings. Ongoing development is important to us, so this is encouraged and supported by a regular monthly half-day closure for team training.
