We have an exciting opportunity for a Recovery Coordinator - Navigator who has the skills and experience to assess and engage with our service users, ensuring that they access the right intervention at the right time, in a way that best meets their needs.
No two days are the same and we treat every person who comes into our service as an individual, so being flexible, curious and a good listener are key.
Where: Base negotiabale but will need to work across Kirklees.
Hours: 37.5hrs per week
Salary : £27,449.52 - £31,529.41dependent on experience
About the role:
- Obtain referrals electronically and in person from third party sources as Hospitals, Mental Health Liaison Teams and GP’s
- Liaise with hospitals as part of discharge planning to effectively support within the community
- Conduct Triage and assessments for all referrals from Hospitals and GP, as well as assessing referrals from GP surgeries and any self-referrals where mental or physical health barriers would impact on access to treatment
- Utilise home visits and community venues for one to one and group work sessions
- Make referrals to statutory and community organisation to address physical and mental health barriers
- Ensure a MDT approach to caseload management to ensure lead agencies are involved and actively supporting service users
About you:
- Have a str ong understanding of substance misuse issues and experience of working within a similar field
- Experience of supporting and advising service users with complex needs to achieve successful outcomes
- Be an excellent communicator, both verbal and written with good IT skills
- Proactive and able to work on own initiative with excellent time management and prioritising skills
- Have a good working knowledge of mental health interventions, services, and good practice
- Be a supportive team player with strong interpersonal skills with the ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholders
- Ability to manage change successfully in a way that prioritises the needs of service users
- Understand the importance of information governance processes and commit to follow and apply all necessary safeguards
- Seek out learning opportunities to improve and broaden your professional knowledge and skills and to contribute and oversee the learning and development of others
