Job Description: Are you a compassionate and dedicated nurse looking for flexible work opportunities in the Bromley area? We are seeking skilled Community Nurses to join our Agency.
As a Community Nurse, you will play a crucial role in delivering high-quality care to patients within their homes, promoting independence and improving overall health outcomes.
Key Responsibilities: - Conducting comprehensive assessments of patients' health needs and developing personalised care plans.
- Delivering a range of nursing interventions, including wound care, medication management, and symptom management.
- Providing emotional support and practical advice to patients and their families.
- Collaborating closely with multidisciplinary teams to ensure coordinated care delivery.
- Maintaining accurate and up-to-date patient records and documentation.
- Contributing to the evaluation and improvement of service delivery through feedback and reflection.
Requirements: - Registered Nurse (RN) qualification with active registration with the NMC
- Previous experience working as a Community Nurse
- Must be able to drive and have access to a vehicle.
- Strong communication and interpersonal skills, with the ability to build rapport with patients from diverse backgrounds.
- A compassionate and empathetic approach to patient care.
- Ability to work autonomously and make sound clinical judgments.
- Flexibility to work across bases if needed
