Job description
Identifying patients at diagnosis for a lifestyle assessment focusing on the pillars supporting treatment optimisation Signposting and referral onto exercise, smoking cessation, weight loss and nutrition programmes Supporting Macmillan Cancer Care Navigators by reviewing Holistic Needs Assessments (HNAs) and formulating Care Plans Identifying patients at risk of poorer outcomes (lower 2 IMD quintiles) to achieve better access to all options for treatment Organising and promoting regular health and wellbeing events for all people living with cancer accessing services at James Paget University Hospital and in the community that it serves Addressing concerns identified in current HNAs including breathlessness, increasing fatigue and lack of mobility and fitness through specialist physiotherapy input Work in partnership with Patient Participation Groups to ensure a strong co-production ethos in service delivery and improvement Please see attached Job Description for full details.
