Job description

Lead and develop healthcare services tailored to the needs of individuals experiencing homelessness or at risk of homelessness. Provide advanced nursing care, including assessment, diagnosis, and treatment, to improve health outcomes and address complex health issues. Collaborate with multidisciplinary community-based teams to ensure coordinated and holistic care delivery. Engage with local stakeholders, including housing agencies, social services, and community organisations, to advocate for the health needs of marginalised populations. Contribute to the development and implementation of evidence-based practices and interventions to address health disparities and promote health equity. Required Qualifications: Masters degree in nursing with specialisation as a Nurse Practitioner. Current registration and licensure as an Advanced Nurse Practitioner with the relevant regulatory body. Extensive experience in managing multidisciplinary community-based teams. Demonstrated clinical expertise in providing healthcare services to vulnerable populations, preferably individuals experiencing homelessness or at risk of homelessness. Strong leadership skills with the ability to inspire and motivate teams toward achieving common goals. Excellent communication, interpersonal, and problem-solving skills. Desired Qualifications: Certification in a relevant specialty area (e.g., Primary Care, Mental Health, Addiction Medicine). Experience working within the field of homelessness or with marginalised populations. Knowledge of local resources and support services for individuals experiencing homelessness.
