EALING HOSPITAL CONSULTANT ADVERT
This is a new post providing an exciting opportunity to expand and develop specialist palliative care services and end of life care services in an acute hospital setting, The successful applicant will have the responsibility for leading specialist palliative care at Ealing Hospital. The post will help integrate specialist palliative care provision across Ealing Hospital, Meadow House Hospice (MHH) and the community specialist palliative care services providing seamless care to local patients.
The post holder will have the opportunity to contribute to the strategic direction of our service, through strong medical leadership and will influence excellence in clinical care, by creating collaborative working relationships across both professional and organisational boundaries, to include local and regional stakeholders within the Integrated Care System.

The post is primarily based in Ealing Hospital where you will be expected to;
1. To provide consultant level specialist palliative medicine expertise to palliative care patients at Ealing Hospital.
2. To provide support for medical and surgical colleagues with complex decision making and discussions with patients with palliative and end of life care needs.
3. To play a role in improving the palliative care for patients in Ealing Hospital, Meadow House inpatient unit, community and other care environments and improving continuity of care as patients transition between these environments.
4. To work alongside other specialist palliative care personnel to continuously improve the skills of Trust staff of all disciplines
5. To contribute to the palliative care team’s education and training programme for healthcare professionals on palliative care topics r
6. To participate in the managerial and strategic work of the service, contributing to the constant modernisation and quality improvement of the service

This is currently a 10-session post with a split of 75% DCC and 25% SPA time. The post could be adapted to suit a less than full time post holder. The post holder would be based at the Ealing Hospital site supported by Meadow House hospice. This is a new post, and the post holder will be instrumental in the future direction of its development. The Ealing community specialist palliative care team collectively saw approximately 800 patients over a 12-month period (42% malignant, 58% non malignant). The new post holder would be expected to contribute to the care of this patient group supported by the hospital specialist palliative care nursing team and the consultants at Meadow House Hospice.
Clinical Duties
The post holder will be required to:
1. work as part of the existing specialist palliative care team at Ealing Hospital.
2. carry out holistic assessment of the needs of patients and those close to them, advice on management of symptoms and other concerns, and advice on appropriate transfer to other settings (e.g., home, nursing home, hospital).
3. act as a resource and support to all members of the multidisciplinary team involved in patient care, focussing on collaboration through excellent communication.
4. ensure that all records are kept in accordance with the Trust’s record keeping policy and in line with General Medical Council’s Good Medical Practice guidance.
5. provide clinical supervision for trainees and will liaise closely with the Educational and Clinical Supervisors of these colleagues, with the aim of assuming these responsibilities.
6. support other members of the specialist palliative care team in managing patients with complex needs and in handling difficult consultations.
7. take part in the appropriate referral of patients to other services, and in handover and sharing of appropriate information with other healthcare professionals including primary care professionals and colleagues in the hospice, other hospital trusts.
8. attend the weekly specialist palliative care hospital MDT and be responsible for ensuring that the medical input into that meeting is available, up to date and recorded accurately.
9. to participate in other palliative care MDT meetings such as the Meadow House inpatient MDT and Ealing community MDT.
10. be flexible in substituting for medical colleagues (all grades) in order to cover both foreseen and unforeseen absences throughout the clinical team at Meadow House Hospice.
11. Collaborate with end-of-life care colleagues to improve the end-of-life care experience for patients and those closest to them within the Trust.
Over time, the post holder’s roles and responsibilities would be expected to evolve, within LNWUHT in order to contribute to management, strategy, service provision and to gain further experience and skills in community and hospice settings.
