An exciting opportunity has become available to work within Care4CE Community Reablement Services. The position is for one permanent Grade.6 Service Co-ordinator 37hours Monday to Friday 9am – 5pm working in the office at Macclesfield Town Hall.
Starting pay £13.69per hour - £26,421.00 per annum – incremental for 5 years plus a yearly pay rise.

An exciting opportunity has become available to work within Care4CE Community Reablement Services. The position is for one permanent Grade.6 Service Co-ordinator 37hours Monday to Friday 9am – 5pm working in the office at Macclesfield Town Hall.

ABOUT THE ROLE
Reablement is a short-term service working with our integrated health partners in supporting hospital discharges and hospital avoidance and includes referrals from the Community Social Care Teams. The service covers all areas of Cheshire East Council.
These are a fast-paced, busy posts managing the referral process and customer journey, attending daily discharge meetings, and working alongside our senior and admin teams as part of a duty desk in the office.
You would be actioning referrals working on our Social Care and internal computer systems, loading the customer data, completing all the allocation of scheduled visits for each customer and templating, including the weekly rostering of visits. Working closely with the Hospitals, Urgent Community Response/Virtual Wards, Social Workers, and Brokerage Teams when ongoing needs are required.
You will also be required to keep various data spread sheets updated with all customers information.
