This is an exciting opportunity to work with individuals experiencing a mental health crisis who may also be at risk of psychiatric inpatient admission. The Social Interventions Collective (SiCol)is a team based in a busy Crisis Home Treatment Team that is promoting the bio-psycho-social model of mental disorder. Your role will be intrinsic to promoting the social model of practice. Your work will be stimulating and challenging and you will work very closely with an experienced team of Social Workers, Approved Mental Health Professionals, Psychiatrists, Nurses and Support Workers. You will be expected to:- have a prominent role in supporting individuals requiring vital and urgent social support complete personalised assessments of social need and identify and arrange social interventions accordingly that specifically aim to achieve recovery complete assessments under Care Act (2014) work closely with a whole range of Statutory and Third Sector colleagues to assess, identify and arrange interventions based on social and recovery models. offer a compassionate and respectful response that inspires hope and recovery This post is exempted under the Rehabilitation of Offenders Act 1974 and as such appointment to this post will be conditional upon the receipt of a satisfactory response to a check of police records via Disclosure and Barring Service (DBS). Who are we looking for? If you are ready for the challenge you will need to demonstrate: you practice with authenticity, commitment and respect at all times you are an excellent team player you have a working knowledge and understanding of the common social issues that impact on an individual experiencing acute and/or enduring mental disorder, and the range of interventions that can be made available to address those social needs you have an understanding of assessing risk and how to escalate this to relevant professionals and services you have the ability to explore, identify and implement creative social solutions to prevent the need for a psychiatric inpatient admission, including knowledge of the creative use of direct payments you have the ability to identify and effectively mobilise relevant agencies, professionals and resources that are central to an individuals recovery plan, you have the ability to form robust and purposeful relationships with individuals, families, carers, and the partner agencies you work with.
