Job description
Main duties and responsibilities Responsible for the coding of clinical documents going into patient records. Following the Practice protocol and determine whether the document needs to be seen by a Clinician for further action or whether it can be filed directly into a patients record. Responsible for the administrative processes behind disease registers and the recall of patients (e.g. asthma). You will support lead clinicians to develop and improve systems to ensure all patients are invited in a timely manner by email, text or phone. You will be responsible for monitoring your domain requirements for QOF/IIF and reporting progress regularly throughout the year. Review clinician coding to ensure appropriate QoF codes are used to ensure QoF targets are met prior to year end. Summarise patient records, ensuring patients have an up to date health record (including summary care record). This will involve scrutinising the medical record for errors, misfiled data, omissions, prescribing and irrelevant or out of date information using both the GP2GP record and Lloyd George record. Record all diagnostically relevant information including allergies and results of investigations. Ensure the Duty Doctor is able to view test results on a daily basis. Process patient subject access requests (SARs) and prepare copies of medical records to be sent to external parties, redacting 3rd party information as appropriate. Process patients medical leave of absence requests following University of Sheffield guidelines. Develop an in-depth understanding of the Practices IT systems to ensure you have an excellent understanding of running complex searches and reports, in order to compile and analyse standard patient data for the Practice as required. Act as an IT Champion within the practice, supporting colleagues with any IT issues (hardware/software) that they experience and liaise with colleagues from the NHS, University IT Service Desks and system suppliers. Assist with NHS Contract Requirements, including Enhanced Services and QOF, to maximise contract income. In addition, work with the Data Quality Team to develop/improve processes within the Practice. Undertake an active part in the preparation of Intro Week and patient registrations, including the screening of medical questionnaires and recruitment/supervision of temporary staff members. Carry out other duties, commensurate with the grade and remit of the post.
