We are looking to recruit three Health Visitors to join our friendly and encouraging across the borough, working full time and part time. Flexible working considered. This is an excellent opportunity to contribute to build the integrated 0-19 Service to support the delivery of the Healthy Child Programme to families in Hillingdon The aim of the service is to achieve equity of health outcomes for the local population through the provision of evidence based practice and needs led service. The post holder will contribute to the identification and assessment of child and family health needs and the delivery of a universal core service, targeted interventions and public health interventions to children and their families within a defined locality. Hillingdon is also where town and country meet boasting 800 acres of woodland, country parks, fields and farms, several rivers and the Grand Union Canal. We actively encourage any applicants to come and meet the leadership team. · Working as part of the 0-19 team delivering a high quality, proactive service for children, young people and their families. · contributing to the identification and assessment of child and family health needs and the delivery of a universal core service, targeted interventions and public health interventions to children and their families within a defined locality. · Working closely with local partners, particularly GPs, Children's Centres and the local community and voluntary sector. · contributing to the success of the team by being an effective role model and supervising community nursery nurses, community staff nurses, health visiting assistants and students to ensure they are working to the standards with people using our services deserve.
