Job description
Work with patients and carers to create individualized care plans. Manage own caseload. Engage with patients in various settings (clinics, telephone appointments, wards) tailored to their individual needs. Establish and maintain effective communication and liaison with all members of the multi- disciplinary team, ensuring seamless collaboration. Work with neighbourhood nursing teams, GPs and primary care colleagues to foster a culture of collaborative working across organisational boundaries. Participate actively in case discussions, and multidisciplinary team meetings to contribute your expertise and insights. Utilise your professional judgment to intervene on behalf of patients, ensuring a patient-centric approach to care delivery. Empower and support patients and their families in making informed decisions regarding their treatment and management. Escalate concerns within the nursing and professional structures as needed.
