Key Responsibilities:
- Provide high-quality, evidence-based physiotherapy assessments and interventions to patients within the Rapid Response team.
- Respond promptly to urgent referrals and provide timely support to patients in their homes or community settings.
- Collaborate with multidisciplinary teams to ensure coordinated and holistic patient care.
- Assess and manage complex cases, implementing effective treatment plans tailored to individual patient needs.
- Maintain accurate and up-to-date patient records, adhering to professional standards and regulatory requirements.
- Participate in ongoing professional development and training to stay abreast of current best practices and advancements in physiotherapy.
Requirements:
- Bachelor’s degree (or equivalent) in Physiotherapy.
- Current registration with the Health and Care Professions Council (HCPC).
- Minimum of Band 6 experience within a Rapid Response or similar setting.
- Strong clinical and interpersonal skills.
- Ability to work autonomously and as part of a collaborative team.
- Excellent communication skills, both written and verbal.
- Commitment to professional development and continuous improvement.
