About the role:

Chosen Care Group is currently recruiting Care Manager with minimum of one year experience in domiciliary / extra care service.

You need to have level 5 in Health and Social Care or equivalent, and proven understanding of CQC regulations, inspections and what it means to achieve a ‘Good’ or ‘Outstanding’ rating.

You should be able to draft comprehensive care plan and Risk assessment.

Manage the day-to-day operations of the care service, ensuring that the highest standards of care provided to all patients.

Monitor to evaluate the quality of care provided and ensure that all patient’s needs are met promptly and efficiently.

Carry out regular assessments of patient to identify any changes in their needs and develop appropriate care plans and update regularly.

Train staff and ensure that all staff are adequately trained and supervised to provide the best possible care for each patient.

Audit to maintain accurate records of all patients, care plans and related documentation.

Liaise with family, health care professionals and social workers to ensure the best possible care for each patient.

Ensure that all relevant regulations and policies are always adhered to.

Organise and attend special trainings along with the staffs delivered by professionals in hospitals or other settings in the community.
