We are looking to appoint a full time Social Worker whose role will be to support timely discharges from acute psychiatric and rehabilitation wards. We have three main hospitals in the Cheshire and Wirral Partnership footprint but sometimes people are in out of area hospitals. The discharge Social Worker will need to develop good working relationships with the wards, work collaboratively with health professionals and community social workers. This is a new role to be developed by the successful applicant by putting processes and plans in place that will enable timely discharges from the wards. This is a challenging role and therefore we are ideally looking for an experienced Social Worker with some background in mental health or a Social Worker who is AMHP qualified. High level of organisational skills and the ability to drive change is a must. You will act as the link person between hospitals and community services and support people to return to their lives with the right level of care as soon as possible. You will have access to a number of basis throughout the locality and also have the option of working from home as and when that is appropriate. You will have:

CQSW/CSS/Diploma/Degree in Social Work or equivalent (Essential)
Social Work England Registration (Essential)
Knowledge of and experience working with people under the Mental Health Act.
Knowledge of S.117 aftercare.
Experience of managing high risk, complex cases and safeguarding Adults.
Be required to undertake an enhanced DBS plus Adults Barred List (Essential)
Statutory and policy base of Social Work (Essential)
Appreciation of recent developments in service provision.
Ability to communicate effectively with customers, carers and families.
Ability to cope positively with change and an ability to promote positive changes to make a difference.
Awareness of importance of multi-disciplinary working and an integrated approach.
Ability to carry out assessment and support planning in line with relevant legislation.
You are confident, energetic and enthusiastic as well as sensitive and resilient.
Self-motivating individual with the passion to make a difference in adult social care.
