Job description
JOB TITLE: Practice Nurse REPORTS TO: Nursing Team Manager RESPONSIBLE TO: GP Partnership ACCOUNTABLE TO: GP Partnership PURPOSE OF THE POST The Practice Nurse will be able to assess the nursing needs of patients. They will be expected to provide appropriate care/treatment in conjunction with the GP, or independently, as appropriate and according to Practice policy, protocol etc. The Practice Nurse will advise patients in general health care and minor ailments with referral to General Practitioners when necessary. It is important that all members of staff are prepared to undertake additional duties or relinquish existing duties in order to maintain the efficient running of the Practice and it is expected that requests from the Partners/Nurse Manager to redistribute duties and functions for such changes shall not be reasonably refused. DUTIES AND RESPONSIBILITIES Identify, and manage as appropriate, treatment plans for patients both at risk of developing a long-term condition and diagnosed chronic disease Undertake cervical cytology screening on those who are eligible in line with national guidelines Dressing and wound management, including compression bandaging/hosiery and Doppler/leg Ulcer care. Respiratory Care Asthma & COPD Suture and clip removal. Management of skin disorders. Recognition of mild childhood illnesses. Testing pathology samples e.g. blood, urine. Blood pressure and weight management. Aid/chaperone to GP during clinical procedures, e.g. minor operations etc. Specimen collection e.g. msu, pregnancy tests, occult blood, faeces for O&S, sputum, etc. The Nurse will be responsible to putting labels on samples, form filling and ensuring timely preparation of specimens for collection to laboratory. General observations e.g. BP, pulse and temperature ECG Use and care of nebulisers. The Practice Nurse will be responsible for upkeep and maintenance of medical equipment. Ensure the maintenance of stock levels in the Treatment Room relating to patient care. Vaccinations/Injections The Nurse will administer childhood immunisations as per the child immunisation programme and have signed relevant Patient Group Directives. Travel vaccination schedules and travel advice. Flu vaccination clinics/any other vaccination clinics as deemed appropriate. Any other routine injection e.g. B12. The Practice Nurse will be responsible for all ordering and re-ordering of injections and vaccines. Fridge hygiene and temperature control. Sharps disposal and clinical waste disposal. ADMINISTRATIVE AND PROFESSIONAL RESPONSIBILITY 1. To comply with the Nursing and Midwifery Code Of Professional Conduct and to keep your registration up to date. 2. To comply with the guidelines for the storage, control and administration of medicines. To administer drugs only on signed, written medical authorisation. 3. Participate in the administrative and professional responsibilities of the Practice Team ensuring that the relevant claim forms are submitted in a timely manner to facilitate claims for payment (childhood Immunisations etc). 4. Ensure accurate notes of all consultations and treatments are recorded on the clinical record on the computer. 5. Ensure accurate completion of all necessary documentation associated with patient health care, and registration with the Practice, including relevant clinic lists and call/recall systems. 6. Ensure collection and maintenance of statistical information required for regular and ad hoc reports or for identifying target group patients. 7. Attend and participate in Practice Meetings as required 8. Assist in the formulation of Practice philosophy, strategy and policy. 9. To be responsible for the safe use and security of all Practice equipment used in the treatment/consulting rooms and reporting any defects to the Line Manager. RESEARCH PROJECTS/AUDIT The Practice Nurse will cooperate and participate as required in any research or audit within the Practice as deemed necessary together with assisting with the requirements of Clinical and Social Care Governance. PROFESSIONAL DEVELOPMENT The Practice Nurse will have responsibility to keep up to date and informed about new medical developments and equipment and maintain continued education by attendance at courses and study days as deemed beneficial to Practice development or necessary for professional development. It is a requirement of all employees of the Bosmere Medical Practice to attend relevant courses/training programmes as determined and agreed by the Partners of the Practice to ensure their development in terms of the requirements of the post at a specified time and the demands of the Practice. All Practice Nurses will undergo an annual Job Appraisal where his/her personal development portfolio will be discussed and reviewed. SMOKING POLICY The Practice operates a Smoke Free policy and all employees must comply with this. CONFIDENTIALITY As an employee of the Practice or following the termination of their employment with the Practice shall not disclose other than to an authorised person or in the course of duty, without lawful authority, any matter or information which they have obtained or to which they have had access owing to their official position. In addition, you shall be required to comply with the requirements of the Data Protection Act in so far as the Practice is concerned. It should be noted in this connection that any unauthorised disclosure of information covered by the Act is a criminal offence.
