The Berkshire Specialist Perinatal Mental Health Service is seeking to recruit enthusiastic, and clinically confident registered Mental Health Nurses, Social Workers & Occupational Therapist, or individual with equivalent qualification licensed to assess, plan, and evaluate care for patients with mental health problems to join our service as a Care Coordinator for Reading area.

Although experience in the field in Perinatal Mental Health is desirable, keen interest in the specialist area is essential. We are a friendly and supportive team and provide extensive support and training for all newly recruited staff. Newly qualified nurses are able to apply as band 5 as a developmental role with a view of promotion to band 6 after completion of preceptorship.

Our service covers the entire Berkshire and there is flexibility of working between home and base. The Perinatal service is commissioned to work with women/birthing people pre-conceptually, in pregnancy and postnatally where there are severe and complex mental health concerns. Our care coordinators operate remotely but also work together with the multidisciplinary team including psychiatrist, psychologist, CBT therapist, social workers, and senior support workers.

Working Hours: Monday to Friday 09:00 -17:00.

1. Clinical

The post holder will be responsible for the clinical triage of service users who access Mental Health Services during pregnancy and in the first-year post-partum and signposting on to the most appropriate service which may include follow up with the perinatal mental health service.

The post holder will conduct social and health choice assessments involving assessment of risk within the support framework of the perinatal mental health team and in a location acceptable to the service user usually within the service user’s home, using the perinatal assessment document.

The post holder will be responsible for assigning service users to a planned care pathway with a formulation, cluster and care recommendation within 7 days of assessment.

The post holder is required to adopt the principles of the Care Programme Approach (CPA) and work within the framework when assessing, planning and co-ordinating the care and medical treatment that a person requires.

2. Leadership

As an experienced practitioner the post-holder will direct and supervise junior staff and resources within their area of responsibility and take an active participation in clinical quality assurance, patient and customer satisfaction, and contribute to the delivery of learning programmes.

1. The applicant should have range of communication skills including verbal and writing.

2. The individual should be a team player and be able to work within the multi-disciplinary team and also liaise among multi-agencies to meet our patient care needs.

3. The individual should be computer literate and be able to utilise the Trust IT system to maintain patient records.

4. Applicant should be a car driver and use of car within work hours is essential.
