An exciting opportunity has arisen for an experienced social worker, keen to progress, who has a strong interest in supporting young people who are approaching leaving care develop meaningful pathway plans to reach their future aspirations.
You will work alongside a small, committed, and experienced team of Social Workers and Personal Advisors, lead by an experienced and knowledgeable Team Manager who are passionate about ensuring our young people are well supported, have clear well informed plans and a successful transition to adulthood.
You will have access to support from the staying close and life long links teams to offer and enhanced package of support to young people in Darlington who are approaching leaving care.
The team has strong well-established links with partner agencies with joint protocols in place with Housing providers and the Department for Working and Pensions (DWP)
As an Advanced Practitioner you will providing advice, support and guidance to the team and supporting the Team Manager with quality assurance activity, driving performance, supervision, and service developments.
*This is a fixed term post until 31 July 2025.
