Opus People solutions on behalf of Walsall Council have an exciting opportunity for Social Worker East Locality in their Adult Social Care Team.
This Role will be Hybrid with working from home, office based and visiting peoples homes.
The role will include:

• To complete Care Act assessments, support plans and reviews of package of care and care home placements. Also completed carers assessments and support plan.

• Use mosaic IT system to record all work including case notes, assessments, reviews and support plans.

• Work on duty to screen new referrals and respond to telephone calls from services users, carers, and other professionals.

• Complete safeguarding section 42's and Mental capacity assessments.
