Social Services and Health jointly provide early intervention/prevention support to people who identify as having mental health problems.
The purpose is to support peoples’ well-being and promote recovery. They do this within the Primary Mental Health Care team and we are looking for a Social Worker to continue this essential work. Set in a busy team of Health and Social Services workers, you will be supported to work alongside people and their families to identify community based solutions, connect with available resources to enable people to achieve their personal outcomes.
The post holder will work in a variety of community settings with people presenting with factors affecting their mental well-being. The post seeks to identify what matters to people in order to promote their general health and prevent further decline, and will look to promote peoples roles as parents, employees and family. You will be well connected with community assets and seek to make links with these to offer solutions with people. You will support people to co-produce assessments and this includes carers.
Qualification as AMHP is desirable for this post, not essential.
