Remuneration:  £500 PER DAY

Description: LOCUM Dentist required from for 2-5 days (FLEXIBLE) in busy, INDEPENDENTLY OWNED practice near Southampton, Hampshire. 
•    £500 PER DAY
•    50% split on PRIVATE income
•    50/50 labs                                                                                                                                                                         
•    VT training practice
•    Existing well maintained patient list - taking over from Associate who has been with practice for 4 years
•    Fully computerised, modern surgeries - 5 Surgery practice
•    Digital x-rays
•    Rotary Endo
•    Qualified nurses
•    Longstanding support team
•    Hours are 8.45am-5.30pm
•    15 minutes to Southampton, 30 minutes from Portsmouth
•    Available now, start date negotiable.

Compliance requirements:
- Full NHS Performer Number
- Local Area Team confirmation
- GDC certificate
- UK Blood test report showing Hep B immunisation and titre levels, Hep C, HIV and TB results dated within last 5 years 
- Indemnity certificate
- Passport
- Enhanced DBS (within the last 12-36 months)
- Clinical references covering places of employment over past 2 years (minimum 2 referees)
- Degree certificate
- CPR/AED/Medical Emergencies certificate dated within the last 12 months
- IRMER
- PIN number
