Job description


Our heart failure specialist nurse team play a pivotal and dynamic role in the care of our patients with heart failure. This role will be centred around supporting the team in the management of patients on the virtual ward platform. Virtual wards allow patients who otherwise would be in hospital to receive acute care, monitoring and treatment at home. Your role will be focused on ensuring medicines are optimised, managing symptoms, providing lifestyle and self-care advice to patients and carers and ensuring the patient has a comprehensive management plan on discharge. You will be supported by our multidisciplinary team which includes specialist nurses, doctors, pharmacists and links to teams such as cardiac rehab and palliative care.

The team also reviews patients in hospital and in outpatient clinics. You will be taking part in Multidisciplinary team meetings and inputting data into the national heart failure database (NICOR).

You will receive a comprehensive induction programme, will be supported by senior HFSN and follow the BSH competency framework to guide development in your role.

This post is one year fixed term and part time (0.8 WTE) due to funding awarded to the heart failure team. Depending on a candidates experience the post may be considered at a band 6 level which would be full time at 37.5 hours per week.

The main duties of this post will be to to facilitate the provision of high quality specialist care to patients with acute and chronic heart failure. This will involve assessing the individual needs of patients using assessment skills and utilising various investigations. The post holder will need to recommend heart failure medications to improve quality of life and reduce morbidity and mortality. They will be expected to promote self management and provide health education.

A key responsibility will be to supporting the management of patients on the virtual ward platform. Virtual wards support patients who would otherwise be in hospital to receive the acute care, monitoring and treatment remotely in their own home. This includes either preventing avoidable admissions into hospital, or supporting early discharge out of hospital. The role will involve identifying appropriate patients and supporting the use of remote monitoring.
