Job description
To provide a high standard of clinical care ensuring safe and effective assessment, treatment and comprehensive discharge planning using highly developed clinical reasoning skills and appropriate assessment tools. Provide care delivery to vulnerable groups, inclusive of but not exclusively: Mild Learning Disability Looked After Children Survivors of abuse Act as Care Coordinator under the CPA under the supervision of line manager. Work collaboratively, and promote effective working relationships, with members of the multi-disciplinary team, ensuring effective clinical decision-making, internally and with external agencies. Empower patients and their families/carers to influence and use available services, information and skills and act as an advocate where appropriate. Provide full assessment and appropriate follow up care via care planning and clinical pathways, individual and group interventions, in partnership with young people/families/carers and other workers involved in care delivery. Provide therapeutic interventions reflective of CYP IAPT, within relevant care pathways. Work within defined waiting times. Provide an extended hours service, as required. Participate in the planning and delivery of group interventions Provide pre-CAMHS support, advice, consultation, and training to partnership agencies. Work in accordance with relevant childcare, health legislation and professional standards in partnership with wider childrens services and to engage fully with Safeguarding, single assessment and other processes which requires working together with other agencies. Proactively involve young people and families in service development including using service questionnaires to obtain structured feedback. Provide a professional, non-stigmatising service to children and families. Work closely with other colleagues within the service to enable effective transitions to and from other areas. Ensure best practice by incorporating NICE guidance and meeting healthcare standards. Adhere to Trust Policies in consideration of Children in Need and safeguarding issues and in maintaining accurate and effective written records. Use outcome measures to evidence effectiveness of treatments provided. Chair both clinical and non-clinical meetings, as required. Propose changes and developments to service delivery and contribute to policy discussions at both team and service level. Participate in management and clinical supervision. Actively participate in the delivery of training opportunities for staff and students. To be able to travel independently and work flexibly. Manage caseload and service delivery in accordance with, and to contribute towards the attainment of, the Service/Trust productivity requirements, including waiting times. Participate in all clinical governance and audit developments including post registration education, training, and continuing professional development. Provide rapid response to children/young people presenting with urgent and emergency mental health presentations over a 24/7 rota. Support the Access Assessment of new referrals for in-patient beds. Engage with young people to provide mental health and risk assessment, risk management plans, and taking appropriate therapeutic risk for young people following crisis. Provide full assessment and collaborative care planning for implementation by the multidisciplinary team, in partnership with young people/carers and other workers involved in care delivery. Ensure best practice by incorporating NICE guidance and meeting healthcare standards. Provide advice/consultation to school staff and other professionals.
