Remuneration: Circa £60,000+

Description: Part time Performer Dentist required for 3 days a week in a busy, mixed practice near Andover, Hampshire. 
•    FIXED TERM CONTRACT FROM MAY 24 TO JAN 25
•    3375 UDAs available
•    £12 per UDA
•    Mon to Fri - 9am to 5pm - flexible on days
•    Established patient list
•    Excellent private potential
•    50% split on private & all labs
•    Fully computerised surgeries
•    Digital x-rays
•    Full support team with Hygienist & Nurse
•    Free car parking
•    35 minutes from Southampton and 30 minutes from Basingstoke
•    Available May 24, start date negotiable

Compliance requirements:
- Full NHS Performer Number
- Local Area Team confirmation
- GDC certificate
- UK Blood test report showing Hep B immunisation and titre levels, Hep C, HIV and TB results dated within last 5 years 
- Indemnity certificate
- Passport
- Enhanced DBS (within the last 12-36 months)
- Clinical references covering places of employment over past 2 years (minimum 2 referees)
- Degree certificate
- CPR/AED/Medical Emergencies certificate dated within the last 12 months
