Support Practitioner - Grimsby Housing Related Support
Contract:
Key Responsibilities:
- Key member of the Grimsby HRS Team
- To provide a programme of person-centred support to customers referred into the HRS in accordance with contract requirements
- To engage with customers on a weekly basis to support individuals to secure, maintain and achieve independent living in the community
- The co-provision of support plans, risk management plans, crisis intervention and engaging with customers in order to deliver positive outcomes in a service that offers involvement and empowerment for customers
- To provide a high-quality service; ensuring outstanding customer service and service delivery by acting as an ambassador for Humbercare and point of contact for customers. Engaging with customers to design services that work for them and utilising resources within the local area to make a real, quality difference to peoples' lives
- Educated to Level 2 Diploma or equivalent.
- Experience of working in a social care or similar environment, preferably with females.
- Knowledge of Psychologically Informed Environments and Trauma Informed Care Principles and Practice.
- Knowledge of statutory and voluntary support agencies.
- Working knowledge of person-centred, strengths-based approaches to support and care planning.
- Experience of working with others to achieve successful outcomes.
- Excellent communication, interpersonal and problem-solving skills.
- Team working experience, working cooperatively with colleagues in complex, pressurised situations.
- Excellent organisational and time management skills and the ability to effectively manage a caseload.
- Willing to work towards a recognised housing/care/support qualification.
