Join our Transitions Team as a Social Care Practitioner (SCP) and make a significant impact on the lives of adults in need of care and support. In this role, you will play a pivotal part in identifying effective strategies to manage their diverse needs. This will involve providing good quality advice and information as well as connecting them to community resources.

The role

Where necessary, the SCP will work with individuals in crisis, stabilizing their situations and, when needed, strategically planning long-term support packages to address ongoing needs.

This position also involves collaborating closely with caregivers, conducting assessments of basic equipment, and implementing minor adaptations to enhance safety and foster independence within the home environment.

The responsibilities of this dynamic role are highly varied, involving engagement with adults presenting a diverse range of needs and disabilities. Additionally, the role will require close collaboration with caregivers, recognizing their integral role in the care process.

Key requirements:

As a valued member of our team, you will be working alongside dedicated professionals, including Social Workers, Occupational Therapists, and fellow Social Care Practitioners. Supported by a proficient administration team, the role highlights the significance of collaborative efforts. Effective teamwork is at the core of our approach, ensuring a comprehensive and unified provision of care and support.

Furthermore, the role extends beyond our immediate team to involve partnerships with both external partners (e.g., Health, Voluntary Sector) and internal partners (e.g., Commissioning, Safeguarding). Your ability to work well with various stakeholders will be crucial for success in this position.

This role requires the incumbent to have a car available at work to undertake his/her duties as they will be expected to undertake at least 1500 business miles per year. Essential Car User status attracts a lump sum allowance paid on a monthly basis as well as a free car parking space if he/she works at one of the Council’s corporate buildings.

This role has been identified as public facing in accordance with Part 7 of the Immigration Act 2016; the requirement to fulfil all spoken aspects of the role with confidence in English applies.
