Job description
Job Purpose To contribute to a high-quality service by carrying out assessment, goal setting, implementing treatment programmes and evaluation of clients who may have complex and/ or chronic presentations including musculoskeletal, sensory impairment, medical and mental health needs, with support and supervision from senior physiotherapists. To work within the MOSAIC (Musculoskeletal), Integrated Respiratory, Health Reablement (Beds and Home), Community Rehabilitation, Falls and Bone Health Services, this may extend or change to other community services in-line with organisational development. To support patients/clients to improve or maintain their level of function and quality of life and plan future care in an appropriate setting in partnership with multi-disciplinary team (MDT) colleagues, including liaising, supporting, and referring on to health services, voluntary services, and public sector agencies. To participate actively in the services training strategy, including in-service training, organisation-specific and multi-agency training. To be responsible for planning a safe discharge from the service. To act as Key Worker as required. Dimensions Responsible for physiotherapy assessment and treatment for a defined clinical caseload appropriate to the service area. Working in conjunction with and under supervision of a senior physiotherapist and other senior clinical professionals. Lead group sessions where appropriate. Able to fulfil physical demands of role, including manual handling applications. Will be required to travel city-wide for visits/meetings. To participate in rehabilitation treatment programmes where applicable. To carry out training of other members of staff. Support colleagues with the facilitation of student placements of any healthcare professional group and be lead Clinical Educator for physiotherapy student placements once appropriate experience achieved. Key Responsibilities Management To undertake day to day clinical prioritisation and work planning, supported by senior physiotherapists. Leadership To establish positive and mutually supportive relationships with all team members. To promote the roles and skills of physiotherapy staff to other professionals. To contribute constructively to unit and multi professional team working. To demonstrate a can do attitude and support others to do the same. To demonstrate an understanding and application of the CityCare values and 6 Cs of compassionate care within daily practice, and support others to do the same. Clinical Skills To be professionally and legally responsible and accountable for all aspects of your own work including the management of clients in your care. To ensure a high standard of clinical care for the clients under your management, and support other team members to do likewise, implementing appropriate teaching, supervision, and delegation skills where appropriate. To assess for, provide, demonstrate, and fit adaptive equipment using problem solving, and analytical skills. To accept clinical responsibility for a designated caseload of clients, and to organise this effectively regarding clinical priorities and use of time and ability to be flexible and respond at short notice to the needs of the service. Working within a legal framework, gain informed consent to treatment and have the ability to identify through assessment, clients who lack the capacity to consent. To demonstrate a sound understanding of Clinical Governance and Risk Management and apply to a work situation. To conduct and report on clinical risk assessments as and when appropriate. Professional To undertake the comprehensive assessment of clients, including those with a complex presentation using investigative and analytical skills through standardised and non-standardised assessments. Formulate individualised treatment plans with rehabilitation goals using clinical reasoning in consultation with senior therapists as required. This should be based on sound theoretical knowledge, utilising a wide range of treatment skills, and options to formulate a specialised programme of care. To advocate on behalf of clients and carers, where appropriate, overcoming barriers to communication such as cultural or language difficulties, physical or mental health difficulties, using skills of empathy to facilitate or empower the client. To be involved in the development of good practice for physiotherapists within Nottingham CityCare Partnership CIC and Social care under the guidance of senior staff. To complete patient/client records in line with organisation, regulatory body and legal requirements. To represent the service externally and actively participate in the services marketing strategy when required, conducting presentations to a wider audience, including voluntary and public sector agencies. To demonstrate a commitment to evidence-based practice and the use of outcome measures and make recommendations for change. To have a good understanding of the research process and be willing to participate in local research and audit when appropriate. To be responsible for own Continuing Professional Development, keeping abreast of new recommendations, research, and developments, incorporating them into your work as necessary. Attend relevant conferences and external training courses. Support more junior staff to do likewise. To maintain a high level of professional standards and work to the Professional Values and Behaviours' as set by the physiotherapy professional body. Policy To be actively involved in Clinical Governance, implement policies and proposing practice changes. To always comply with organisations Lone Working Policy and local team guidelines when working alone. Planning and Organisation To be responsible for own time and caseload management. To liaise effectively with other professionals and agencies to enhance rehabilitation within the boundaries of Nottingham CityCare Partnership CIC and Social Care. To use information technology daily to record information and statistics, to aid communication and produce reports. To always ensure the Health and Safety of staff and clients in accordance with Health and Safety legislation, Trust policy and procedures. To undertake any other duties which may be reasonably required. Training To comply with and complete statutory and mandatory training in line with Nottingham CityCare Partnership policy. To attend training events as appropriate to support your continuing professional development. To maintain a continuing professional development portfolio identifying learning objectives. To actively participate in your own supervision, annual performance review and personal development plan.
