Remuneration: 50% split on PVT income

Description: Part-time SPECIALIST ENDODONTIST required for 3-4 days per week (FLEXIBLE) in a PRIVATE practice in Cheltenham.
•    50% split on PVT income
•    Well established practice in the community
•    Well established referral list
•    Long standing supportive staff members with a family atmosphere
•    State-of-the-art and modern working environment
•    Fully computerised (SOE) with Digital Radiographs & CBCT Scanner
•    Practice is established and has a great reputation in the area
•    Full support team with Treatment Coordinator and on-site Practice Manager
•    Specialist practice offering Orthodontics, Implants, Invisalign & Endodontics

Compliance requirements:
- Full NHS Performer Number if applicable
- Local Area Team confirmation
- GDC certificate
- UK Blood test report showing Hep B Immunisation and Titre levels, Hep C, HIV and TB results dated within last 5 years 
- Indemnity certificate
- Passport
- Enhanced DBS (within the last 12-36 months)
- Clinical references covering places of employment over past 2 years (minimum 2 referees)
- Degree certificate
- CPR/AED/Medical Emergencies certificate dated within the last 12 months
