Remuneration: Circa £100,000 Take Home

Description: Full or part-time Associate Dentist required for 2-4 days per week (FLEXIBLE) in a busy, mixed, INDEPENDENTLY OWNED practice near Leek, Staffordshire
•    £14 per UDA
•    Take home £7-8000 per month
•    50% split on PVT income
•    50% split on Lab fees
•    Fully computerised surgeries
•    CBCT, Digital X-rays, iTero Scanner
•    Provide Invisalign and Implants
•    Qualified nurses
•    Hygienist and Therapist on site
•    Free on-street parking
•    20 minutes from Stoke, 60 min from Manchester, 40 min from Cheshire
•    Available now, negotiable

Compliance requirements:
- Full NHS Performer Number
- Local Area Team confirmation
- GDC certificate
- UK Blood test report showing Hep B Immunisation and Titre levels, Hep C, HIV and TB results dated within last 5 years 
- Indemnity certificate
- Passport
- Enhanced DBS (within the last 12-36 months)
- Clinical references covering places of employment over past 2 years (minimum 2 referees)
- Degree certificate
- CPR/AED/Medical Emergencies certificate dated within the last 12 months
- IRMER
