Social Worker – Learning Disability Team
Duration- up to 3 months ( subject to review)

The Community Learning Disability Team is a very friendly, motivated and dedicated team of social care staff. Our team undertakes care management and safeguarding responsibilities in line with the Care Act 2014, Mental Capacity Act 2005 and other adult social care statutory duties.

We work closely with a multi-disciplinary health team including Clinical Psychologists, Speech and Language Therapists, Community Nurses, Occupational Therapists, Physiotherapists and Behaviour Analysts to support adults with Learning Disabilities and associated complex needs.

The team provides a transition service by working closely with children social care and Special Education Needs in line with the Children and Families Act 2014 to support young people who would be eligible for adult social care to have a smooth transfer to adult service.
We support adults and their carers’ in a person-centred way to assess their needs and create effective outcome-focused support plans. Responsibilities include reviewing support plans, taking turns to act as a duty officer to cover for unallocated cases, undertaking safeguarding enquiries, liaising with families, advocates, health professionals, providers and key agencies.

An opportunity has arisen for 3 Social Workers to join the team due to a number of vacancies that has recently arisen.
We are looking for an enthusiastic and motivated person to enable adults with learning disabilities achieve the best possible outcomes. The ideal practitioner will have an understanding of the challenges adults with learning disabilities and their families face in their day to day lives.
You would be a qualified social worker registered with Social Work England and have some experience of working with people with a learning disability. You must have excellent communication skills, sound knowledge of relevant legislation and the ability to form and maintain effective working relationships with people.
