Job description
The East Norwich Community Nursing team has an exciting opportunity for a registered nurse to join our friendly and supportive team of Nurses, Health Care Assistants, Assistant Practitioners and Nursing Associates, to provide nursing care and interventions across Norwich Place. We are a well-established team who are passionate about Community Nursing and aspire to give our patients the very best care that we can give.
We treat patients in their own homes, residential homes, supported living complexes, and also provide catheter clinic and leg ulcer clinic services. The kind of nursing care we provide is vast, and includes (but is not limited to) diabetes management, wound care, catheter care, palliative care and post-operative care. Experience is preferred but not essential as full training will be given in each area. A comprehensive induction and supernumerary period will be enacted on appointment.
As a Band 5 Nurse within our team you would be thoroughly supported by your colleagues, and in time you would have the opportunity to show your support to others such as working with Nursing Students and junior colleagues. At NCH&C we are passionate about furthering the development of our staff and you would have ample opportunity to attend specialised training and courses to facilitate this.

Key responsibilities include
• To assess, prescribe and deliver care packages for patients. To work within the integrated team to facilitate early discharge from hospital.
• To work within the integrated team to prevent unnecessary admission to hospital.
• To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users.
• To process referrals though clinical assessment, evaluating the information received to prioritise a response and allocate to the appropriate service.
• To liaise and communicate with the general public, other health care professionals, and multi-agencies on a daily basis.
• To provide clinical assessment for incoming nursing referrals.
Norwich locality is working alongside it's commissioners and local service providers as part of the Primary Care Network to develop community based services that support patients to remain at home and receive the services they require in an effective and co-ordinated way.

[email protected] The workload requires a good range of clinical skills to be applied in managing our patients with varied health problems including diabetes management, traumatic and post-operative wound care, pressure ulcer prevention and management, catheter management, bowel care, and palliative and end of life care.
Many of our patients will have complex multiple pathologies and so we aim to provide holistic, patient centred assessments, and deliver individualised care.
