Job description

• In conjunction with the team of anticoagulant specialist nurses and the lead haematology consultant for anticoagulation therapy, you will be responsible for developing the anticoagulant service at Frimley Health.
• Provide skilled and competent care to meet patient’s anticoagulation needs involving other members of the health care team as appropriate.
• Ensure the provision of continuity of care.
• Assessment, planning and implementation of the care of patients who require anticoagulant therapy in accordance with agreed protocols.
• Assist in the development of anticoagulant care pathways, in order to facilitate a seamless delivery of care, acting as a key worker for their pathway.
• As a leader in the field of anticoagulation , make sure that each patient’s treatment and care is based on best practice and evidence.
• To liaise with patients and GPs and organise implementation of planned actions deceided and agreed with the lead Haematology Consultant for anticoagulant therapy.

• Promote Nurse led service to Patients, Families, Carers, Community Nurses and General Practitioners.
• Consult with Medical staff, Haematologist’s and General Practitioners regarding their patients anticoagulant care pathways..
• Provide a link in the care of patients with anticoagulation needs between Acute and Primary care and other MDT’s.
• Monitor and evaluate care regarding to the anticoagulation therapy prescribed.
• Interpret blood test results in order to safely advise patients regarding their anticoagulant therapy as prescribe as required.
• Assess patients to be able to safely administer their own anticoagulant medication.
• Provide education and information , both written and oral , to patients receiving anticoagulation therapy as per Frimley Health Guidelines
• Liaise with medical staff regarding other blood test results that may affect the INR.
• Issue advice to patients and or medical staff in preparation for a medical procedure.
• Issue advice to Dentists in the community regarding patients who are to receive invasive dental treatment.
• Note patient’s concurrent medication and advise against any adverse reactions.
• Monitor and evaluate patients receiving anticoagulant therapy via postal clinic.
• Manage own caseload on receipt of anticoagulant / DVT
• Maintain accurate up to date records of all contact with patients and their carers, using appropriate software or in the patient’s medical records.
• Gain skills in obtaining capillary blood samples and venepuncture.

• Organise , coordinate and deliver nurse led anticoagulant clinics across Frimley Health.
• Receive new referrals from Medical staff, General Practitioners and other hospitals.
• Continue to update own knowledge regarding anticoagulation therapy.
• Work within the guidelines of Patient Group Directions.
• Attend education program and form a link with other anticoagulation nurses to encourage sharing of information.
• Participate and lead in clinical audits in respect of all areas of anticoagulation therapy.
• Act within the National and Frimley Health NHS Foundation Trust , Anticoagulation Therapy guidelines.
• To represent the anticoagulation service at local safer medication committee meetings and present reports / findings where appropriate.
