Job description


An exciting opportunity has arisen for a dynamic and enthusiastic individual to join the award-winning Diabetes & Endocrinology Team in the role of a Band 5 Diabetes Nurse.

The Diabetes Nurse will support the wider multidisciplinary team which has a strong commitment to improving standards of care locally. We are looking for a compassionate, highly motivated nurse with a keen interest in diabetes care.

The main focus of the role involves ensuring the smooth flow and preparation of daily Diabetes and Endocrinology clinics whilst supporting the senior members of the team with clinical assessments; there is also the opportunity to support with facets of patient education.

The role will typically require the Diabetes Nurse to work at the St Helens hospital site but on occasions may require travel to other hospital sites in the area. The role will typically be working Mon – Fri although you will need to be flexible and on occasions willing to support with evening clinics or weekend working as needed. Training and support will be provided to the successful individual.

Interview Date - 21st March 2024
• To undertake the role of Diabetes Nurse within Diabetes & Endocrinology outpatient clinics to ensure the smooth and safe running of clinic sessions.
• To assist the clinician with any process and procedure necessary for the examination, investigation and treatment of patients.
• To undertake patient assessments including documenting drug lists and other Diabetes & Endocrinology assessment criteria as per clinic paperwork.
• Provide appropriate education and advice to people with diabetes or staff within the Trust which may include things such as Diabetes Group Education, Insulin Education, Blood Glucose Monitoring Education and Health Promotion information both verbally and written.
• Administer drugs and other treatments as necessary according to policy.
• To conduct tests and investigations according to trust policies.
• To liaise with staff and departments about patients conditions and needs.
• To provide leadership and supervision to the Diabetes HCA and outpatient clinic staff working within the diabetes centre.
• To ensure all blood pressure monitoring and other screening methods used for diabetes clinics is conducted to a high standard and is accurately reported.
• To complete any necessary documentation in accordance with Trust policy and NMC guidance.
• To act at all times in the interest of patients and their families ensuring that a quality service is provided.
• To expand and maintain existing skills to meet patient service delivery.

KEY DUTIES

• To expand and maintain existing skills to meet patient service delivery.
• To take messages from the nurse answer machine or other appropriate sources, and within their competence provide telephone support to people with diabetes.
• Ensure all patient equipment and supplies (including drugs) are in stock and in date as per the stock rota checklist.
• Ensure the resuscitation trolley and other key equipment and departmental checks are maintained according to Trust policy.
• To undertake appropriate Link Nurse roles for the specialist team and any other link nurse or training roles deemed appropriate.
• Support with general housekeeping, stock rotation and other appropriate roles within the Diabetes Centre.
• Support with the development of paperwork, educational resources and leaflets within the specialist team and ensure stock is available at all times.
• Undertake any appropriate tasks to support senior members of the team in the running of the Diabetes Unit.
• Support the patient drop-in service as needed, within levels of competence.

CLINICAL & PROFESSIONAL RESPONSIBILITIES:
• Maintain Professional Registration
• Adhere to relevant Code of Practice of Professional body
• Ensure that nursing care is appropriate, provided and is consistent with clinical guidelines and standards
• To report and record any untoward incidents
• To assist in the promotion of effective communication with all staff, patients and relatives and other departments as required
• To have responsibility for the security and maintenance of equipment belonging to the department. Help to ensure that all equipment is in safe working order in conjunction with the coordinator for this role
• To assist the Senior Diabetes Team in achievement of key performance indicators for the department and the Trust

ADMINISTRATIVE RESPONSIBILITIES :
• To utilise knowledge or hospital systems/procedures in order to appropriately divert/refer patients to alternative/appropriate services and pathways.
• To ensure that the recording of patient care is comprehensive, concise and in line with the NMC Record Keeping guidance and Trust policy and procedure.
• Ensure Patient Tracking IT system is current and reflective of patients in his/her care.
• Support with maintaining student and new starter timetables within the department.

TEACHING & TRAINING RESPONSIBILITIES :
• To personally ensure fulfilment of the requirements for professional updating and to maintain a personal record for future registration purposes.
• To participate in the teaching of students and the in-service training of qualified nurses and health care assistants and act as facilitator/mentor/preceptor as appropriate.
• To support education for diabetes patients including structured education programmes ran by the Diabetes Centre.
• To keep up to date with any required competencies required to support Diabetes Structured Education courses.

LINE MANAGEMENT/SUPERVISORY :
• To act as a preceptor/mentor for newly qualified staff nurses
• To act as a role model to junior colleagues and students and a support mechanism for senior colleagues

FINANCIAL RESPONSIBILITIES
• Support with ordering of essential supplies for Diabetes unit via SBS or appropriate methods

RESEARCH & AUDIT
• To support the Diabetes & Endocrinology Governance MDT agenda
• Attend weekly department MDT meetings
• Contribute to the specialist team audit programme as appropriate
• Contribute to the team-teaching sessions as appropriate
• To participate in research/special projects/audit as requested by the Diabetes and Endocrinology Team
• Contribute towards the setting, maintaining and monitoring of standards of care within the department by participating in department audits.
