Job description
- To use highly specialist clinical reasoning skills and knowledge to assess, diagnose, plan and offer a range of treatment options; deliver, evaluate and amend the treatment plan working as an autonomous practitioner. Contribute to the medical diagnosis and management of care of the elderly patients, who may have diverse or complex presentations/multi-pathologies in individual or group therapy session. - To communicate effectively with patients and carers potentially complex, sensitive or distressing information; in order to provide patient centred care, to encourage self-management, maximise rehabilitation potential and to ensure understanding of the condition. Patients may have communication, perceptual or cognitive/understanding difficulties - To exercise a high degree of professional autonomy, creating and developing protocols/guidelines for the management of patients. To make recommendations regarding national guidelines, initiatives and research. Lead best practice by the use of validated assessment tools and review of intervention outcomes in order to ensure a high quality evidence based service is provided. - To be responsible for operational management of the care of the elderly therapy team by monitoring, coordinating own and other caseloads, including reallocation of work during absence and appropriately delegation to junior therapists and support workers.
