23 months contract with a North London based Local Authority

Job Summary:

This is an exciting opportunity to be based within a local authority integrated health and social care service. The service comprises of many disciplines including Social Work Community Nurses Psychologists Occupational Therapists Speech and Language Therapists Art Therapists Physiotherapists and Psychiatrists. The Care Coordinator post will sit within the social work team.

The role entails face to face work with service users their families and external partners. The purpose of the Care Coordinator role is to identify whether support packages continue to meet the needs of service users and their families.

The role will require the successful candidates to make recommendations regarding support packages and arrange and amend support to ensure needs are being met in a person centred and strengthbased way. Clear vision of meeting needs in cost effective manner in line with the strengthbased approach.

The role also requires the successful candidates to be part of our integrated front door duty service on a rota basis supporting with resolving crisis situations.

An exciting part of the role will include undertaking assessment work and supporting with safeguarding case work.

You will need excellent communication skills both verbal and written. The Care Coordinator will be required to attend multi disciplinary meetings and present cases communicate effectively with service user s and their families as well as a range of internal and external colleagues and partners. In addition the Care Coordinator will be required to write accurate comprehensive support plans and assessments.

Key Duties/Accountabilities:

To undertake a quality assurance role in relation to the quality of the service as experienced by the customers by:

Undertaking timely reviews of service outcomes and the services provided.

Ensuring care packages are meeting the outcomes of service users and continuing to provide value for money.

Inform procurement managers and commissioners of any quality issues or gaps in the market.

Liaising with other professionals as appropriate.

Essential Experience Required:

Ability to communicate clearly and effectively both orally and in writing including report writing presenting negotiating and the use of IT.

Ability to work in partnership with carers and families and liaise with other staff agencies to ensure that the needs of the service user are met.

Experience of working in a health or social care setting with a range of adults with different levels of needs.

Relevant knowledge of legislation policies and guidance that underpin the service.

Knowledge of resources to support the provision of services and to promote independence (e.g. Telecare equipment and other interventions).

Ability to operate Care First and Eclipse (Desirable).

Trusted Assessor trained (Desirable).

Additional information to note:

Trusted Assessor will be a requirement of the role. Training will be provided if the applicant is not currently at that level.
