Job description
Digital Transformation Lead the PCN Transformation agenda and support integration with partner agencies. Lead on the development of the PCN and manage accordingly. Keep abreast of current affairs related to PCN business and proactively identify potential opportunities and threats and develop ideas for future practice and PCN development. Draft business plans, share for comment and feedback and submit to commissioning authority. Improve adoption and/or optimisation of new technology to enhance patient access and experience and increase PCN productivity. Build relationships and facilitate collaboration between practices and the wider system to support the delivery of care to patients (including shared appointments between practices to aid delivery of enhanced access) Lead an improvement approach to change including building capability for quality improvement within the PCN and system wide approaches to problem solving. Review and improve the PCNs digital maturity. Use data, and improve data quality, to: Understand demand, capacity, and activity. Drive improvements in population health and staff Wellbeing at work. Support understanding of the type and intensity of support/training needs of the PCN and coordinate this support, including through OD programmes. Facilitate clinically led innovation and the effective adoption of improvement initiatives, including integrated working at neighbourhood and/or place level to improve access to services for patients. Workforce Management Lead on the development of the PCN Workforce Plan and manage accordingly. Support PCN on all matters related to Human Resources (HR) including: - Supporting recruitment, induction, training of new PCN staff (Additional Roles Reimbursement Scheme), line management, performance appraisals, one to ones with PCN staff. Work as a flexible member of the PCN, providing support to other team members and GP Practices when necessary. Take an active role in the development and embedding of the PCNs culture, values, and reputation as a provider of high-quality services. Encourage PCN people to collaborate efficiently and effectively sharing information and intelligence across different activities. Provide coaching and support to PCN people and stakeholders involved in PCN transformation and development. Support with the development of the PCN so that it is resilient for the future. Lead on the estates plan. Maintain effective and timely communication with the Clinical Directors, PCN team, Practice Managers, practices, ICB, and other stakeholders to promote the ethos of the PCN. Encourage supporting people to collaborate through sharing information and intelligence across different activities. . To support Operations Manager in delivering Development of the PCN Operational Plan and manage accordingly. Contractual requirements on behalf of the PCN. Work with GP practices within the PCN to identify areas of common concern or potential for collaborative working to improve the resilience of general practices, sustain local services and support recruitment and retention of key members of the primary care workforce. Ensure the PCNs compliance with the Data Protection Act, Freedom of Information Act, Information Governance, GDPR, Equality Act and confidentiality. Ensure the PCNs compliance with the GDPR and FOI along with other relevant legislation and professional information management standards such as GMC/BMA and LMC guidance. Alert PCN members to issues of quality and risk. Oversee the organisation of PCN Board Meetings and any other PCN stakeholder meetings as required. Development of the PCN Finance and Governance Plan and manage accordingly. Compile and present financial reports after reconciliation. Ensure financial risks are recognised and appropriate action taken. Monitor expenditure and identifying significant deviations from plan. Plan and monitoring income and cash flow to ensure that income is maximised and that funds are available as required. Ensure the practices allocation of resources is fair and equitable. Report monthly to the PCN Board the financial positioning (including budgeting, forecasting and scenario planning). Ensure the PCN is aware of contract financial responsibilities and tax liabilities. Facilitating end of year accounts. Provide expenditure details to the PCN Board, highlighting variances from the project budget. Fully utilise, efficiently and effectively the PCN development funds and resources to help the PCN achieve its strategic objectives. Making effective use of training to update knowledge and skills. Reporting potential risks identified. INFECTION CONTROL: All staff have both duty and responsibility to avoid and prevent the spread of infection as a consequence of any actions or activities they are carrying out during the course of their duties. The post holder must: - Take all reasonable care to minimise the risk to service users, colleagues, and members of the public (including themselves) of healthcare-associated infections. Co-operate with the PCN in ensuring that all statutory and mandatory requirements are complied with- in particular, compliance with the Health and Social Care Act 2008: Code of Practice for the prevention and control of healthcare-associated infections. Be knowledgeable of and compliant with, the PCNs infection control policies and procedures, which includes attendance at mandatory training. DATA PROTECTION: It is a requirement of employment within Newcastle North PCN that all staff must comply with the obligation of confidentiality to personal identifiable information. The Data Protection Act 1998 safeguards the handling of information held in both electronic and manual filing systems and it is the duty of all staff employed by the PCN to uphold the principles of the Act. FREEDOM OF INFORMATION ACT: The Freedom of Information Act 2000 requires all public organisations to provide any member of the public, upon receipt of a written request, with the information they require, subject to certain exemptions. Every member of staff employed by Newcastle North PCN must take responsibility, in conjunction with the PCN Manager, for any request for information that they receive and respond in line with the PCN Freedom of Information Policy and guidance.
