Job description


This is an exciting opportunity to be part of a new service within the CMHT's in Trafford. You would be working alongside a B7 to help manage the Waiting well service across both CMHT's in the Trafford Area, North CMHT in Sale and South CMHT in Altrincham. Travel across all areas of the brough will be expected.

The teams have a supportive approach for staff with a range of resources available including regular supervision, both clinical and managerial, annual professional development reviews and regular learning opportunities from a range of professionals and training opportunities.

The teams are friendly, welcoming and the role allows professionals to work autonomously but also within a large MDT, which allows for flexibility in the role.

Trafford Community Mental Health Teams are a committed integrated service that strives to provide quality recovery based support to people with severe and enduring mental health related difficulties in Trafford 7 days per week, 365 days per year.

The service calls upon a wide range of evidence based holistic assessments and interventions.

As a care co-ordinator working in the service you will be required to work to help ensure the safety and well being of the service users under the waiting well service in line with CPA process, you will have a complex caseload which could consist of people who are on the Assertive Outreach pathway, and will be expected to work proactively and flexibly within a multi-disciplinary team model.

There is a requirement to provide clinical leadership and to assist in the organising, supporting, supervision and development of junior staff members.

You will also be expected to act as assessor/mentor for student nurses.

The post holder will play a key role in the further development of the service and in the implementation of its Clinical and Social Care Governance agenda.
