Remuneration: £500 PER DAY

Description: Part time or Full time LOCUM DENTIST required for 3 days per week  in a busy, mixed, well established INDEPENDENTLY OWNED practice near Wokingham, Berkshire.
•            Thurs, Fri & Sat
•            £500 PER DAY
•            PRACTICE OFFERS IMPLANTS & INVISALIGN
•            Fully computerised surgeries  
•            iTero Scanner
•            Implant motors
•            Digital x-rays
•            Full support team including a hygienist
•            Existing, well maintained lists
•            Available now, negotiable start date

Compliance requirements:
- Full NHS Performer Number if applicable
- Local Area Team confirmation
- GDC certificate
- UK Blood test report showing Hep B Immunisation and Titre levels, Hep C, HIV and TB results dated within last 5 years 
- Indemnity certificate
- Passport
- Enhanced DBS (within the last 12-36 months)
- Clinical references covering places of employment over past 2 years (minimum 2 referees)
- Degree certificate
- CPR/AED/Medical Emergencies certificate dated within the last 12 months
- IRMER
- PIN number
