An exciting development opportunityhas become available in the Halton Adult Speech and Language Therapy Team!

Bridgewater CommunityHealthcare NHS Foundation Trust is looking for an enthusiastic, Speech &Language Therapist to join their innovative community speech and language therapyteam.

This is a Specialist Clinical Post in theassessment, diagnosis and treatment of patients with acquired communicationand/or swallowing disorders, including voice and respiratory disorders. Thecaseload is a community caseload.

Staff wellbeing is at theheart of all we do. You will have access to regular clinical supervision, teammeetings and regular training/CPD sessions.We ensure you are well supported and continually reflect on yourlearning and aspirational needs in the role.

The post holder will bebased within an established team of speech and language therapists, speech andlanguage therapy assistants, and administrators.

The team work hard toensure that everything they do is based around patient centred, quality carefirst and foremost, and the ideal candidate will be able to demonstrate thatthey share these values.

Main duties of the job

The postholder will provide specialist speech/language/swallowing assessment, intervention,advice and training to adults, their carers and other professionals andagencies involved with the person.

Theywill be responsible for the prioritisation and independent management of aclinical caseload.

Thepost holder will provide specialist clinical support and advice to other therapistsand work as part of the multidisciplinary team, contributing to development andevaluation of the services provided.

The postholder will provide clinical education to SLT students from local universities. They will have a responsibility for CPD andto support and deliver their work in line with best practice.

Job responsibilities

Job Purpose:

The Speech and Language Therapy (SLT) Departmentdelivers services to:

Specialistcommunity and sub-acute adult caseload including adults with acquired dysphagiaand/or communication impairment (acquired neurological disorders and otheraetiologies)

Voicecaseload including specialist input into a bi-monthly Joint Voice Clinic.

Thepost holder will predominantly deliver services to the Halton population.

The post holder will provide services to adultclients in the community, as follows:

ProvideSpecialist Speech and Language assessment, intervention, advice and training toadults, their carers and other professionals and agencies involved with theperson.

Beresponsible for the prioritisation and independent management of a clinicalcaseload.

Providespecialist clinical support and advice to other therapists

Workas part of the multidisciplinary team, contributing to development andevaluation of the services provided

Provideclinical education to SLT students from local Universities

Havea responsibility for CPD and to support and deliver their work in line withbest practice

Duties and Responsibilities:

Clinical

Tocomplete detailed assessments using knowledge of specific assessment toolsrelevant to the client group, differentially diagnose, develop care plans andidentify goals appropriate to knowledge of prognosis, and plan, implement andevaluate detailed programmes of specialist SLT treatment for the designatedcaseloads.

Toensure that clients, families and other carers are involved in the planning andprioritisation of care plans.

Touse specialist clinical knowledge to inform sound clinical judgement anddecision making for case management.

Toprioritise the caseload following departmental procedure.

Tosupport, supervise and enhance SLTA and volunteer work within the department

Tocarry a clinical caseload within the defined clinical settings comprising: adults with acquired neurological disordersand dementia resulting in a diverse range of communication disorders and/ordysphagia

Toprovide a flexible and person-centred service to these client groups

Todeliver therapy on a 1:1, group and programme/care plan basis.

Touse a range of approaches to meet the needs of individuals, includingreflecting on global aspects of a client/carer communication and adapting theenvironment to enhance communication,

Toadapt practice to meet individual circumstances, including cultural andlinguistic differences.

Touse a consultative model of delivery to enable non-SLT staff/carers to carryout therapy programmes and support communication in a range of settings.

Towork collaboratively with carers to promote the use of strategies/activities inorder to facilitate language learning and communication skills.

Toprovide advice to carers/other professionals and other SLTs on complex speech,language and communication difficulties.

Towrite reports reflecting specialist knowledge.

Todemonstrate clinical effectiveness by use of evidence based practice andoutcome measures.

Tobe responsible for continuing developing knowledge and competence in theability to apply a range of approaches within intervention / therapy.

Tomake discharge decisions involving clients following departmentalprocedures.

Tocommunicate a clients diagnosis e.g. dysphasia / dysarthria to professionals,carers and clients.

Toensure consent of the client; carers, and other professionals is gained,ensuring that their understanding is complete. To take into account theemotional psychological and / or physical conditions / presentations andimplications.

Toensure the involvement and motivation of a communication impaired client inunderstanding and participating in the assessment, diagnosis, intervention andtherapy process.

Inliaison with clients, to use relevant supportive communicative skills such asusing pictures, signs, gestures, AAC technology and simplified languageappropriate to the clients needs.

Toparticipate in team meetings as appropriate.

Toliaise regarding specific clients and client groups and work jointly with othermembers of the multi-disciplinary team

Toprovide up-to-date advice and information to the multi-disciplinary team,family and carers, regarding specific clients communication needs and whatstrategies are advised to enable effective communication.

Towork closely with clients, carers and families in agreeing care plans for therelevant management of the client.

Toliaise as appropriate with other members of the SLT team, including peers,managers, assistants and admin & clerical staff intervention and therapyprocess.

Torecognise potential breakdown and conflict across a range of clinicalsituations for example in nursing homes with nurses and families etc. and seekto resolve it.

Torelay complex and sensitive assessment findings to carers and otherprofessionals.

Todemonstrate empathy where appropriate

Toform productive relationships with others who may be under extreme stress and /or have challenging communication difficulties.

Tomake onward referrals as appropriate.

Toattend and contribute to case conferences, annual reviews andmulti-disciplinary team meetings relating to specific clients on the caseload.

Tobe aware of and access more specialist support relating to individual caseswhere necessary.

Toprovide written and verbal advice detailing the needs and recommended futuremanagement of clients

Training

Toprovide training to other members of the multi-disciplinary team, families,other AHPs, medics, nursing staff and voluntary groups.

Toprovide training to meet the needs of staff in discussion with others,including voluntary agencies, nursing homes etc.

Toprovide training to groups of carers where appropriate.

Toparticipate in development and delivery of specialist training to other SLTs.

Toprovide feedback, pass on ideas and attained skills, and to demonstratereflective practice to SLT teams and the MDT, following training received.

Toattend clinical supervision in line with departmental policy.

Toattend relevant training and Special Interest Groups relevant to post holderand service needs.

Toattend specialist short courses appropriate to clinical specialism

Toprovide feedback to highly specialist therapists, peers and non-specialists onspecialist post-graduate courses.

Toparticipate in peer review as appropriate and the annual appraisal system.

Toparticipate in the development of local clinical guidelines informed byevidence within clinical specialism.

Tosupervise the work of volunteers, assistants and students

Toactively identify areas of personal and professional development to be includedin a PDP (Personal Development Plan)

Professional

Thepost holder has responsibility to continued professional development and tosupport and develop their clinical work in line with best practice.

Tomaintain comprehensive and contemporaneous clinical records in line withnational and local standards.

Towork on local policies and procedures regarding client group management.

Toadhere to National (RCSLT and HCPC) and local standards and guidelines relatingto clinical and professional practice.

Todevelop a working knowledge of relevant procedures including safeguardingchildren, working with vulnerable adults and other legal frameworks.

Tobe aware of and adhere to the current legislation related to the client groupstreated.

Tobe aware of the Trust complaints policy and procedure

Respondsensitively and appropriately to initial complaints.

Toinform the Manager immediately of any likely and / or actual complaints.

Tokeep up to date with current trends, literature, research as appropriate

Todemonstrate specialist knowledge in clinical specialism underpinned by currentevidence based practice

Toadhere to Trust policies re: health and safety, security including dataprotection, infection control, child protection and risk assessment.

Tomaintain HCPC and RCSLT registration

Toinform Manager of any difficulties in ability to maintain or achievedepartmental or professional standards.

Tobe accountable for own professional actions and recognise professionalboundaries seeking advice as appropriate.

Towork independently, accessing appraisal within the KSF/PDR framework atdetermined intervals

Todemonstrate the ability to reflect on practice with peers and mentors andidentify own strengths and development needs.
