2. Job Purpose To provide professional leadership including line management, recruitment, supervision/ appraisal and development both qualified and unqualified staff within the Community Team. To provide an expert and evidenced based Nursing service to patients requiring nursing care within their own homes. To work autonomously and as a member of the multi-disciplinary team, developing and maintaining effective working relationships with other agencies including acute, primary and voluntary sectors. 3. Dimensions To work as an autonomous practitioner and be professionally and legally accountable for all aspects of their professional and clinical work, within agreed national, local and professional standards and guidelines. To assess the health needs of clients, formulating and implementing evidenced based nursing interventions based on best practice, knowledge and experience. Develop a comprehensive and concordant plan of care with the client and/or carers, promoting choice and empowerment of the client. To monitor, develop and maintain a high standard of clinical practice both for self and other nursing staff within the team. To provide leadership within the team and to manage conflicting and competing priorities both clinically and managerially. 4. Key Result Areas Clinical To assess, identify and manage risk within the community setting. To be responsible for maintenance of timely and accurate written and electronic patient records in line with professional and Your Healthcare protocols and guidelines. To act as advocate for the client to ensure health needs are met with the consent of the client. To develop and maintain links with the district nursing service and promote integrated working with other community agencies to ensure an optimum and seamless patient pathway which delivers flexible and timely care To promote partnership working with acute sector staff to ensure safe and effective discharge of patients from hospital to community, attending discharge planning meetings as appropriate. To provide appropriate care of patients requiring intravenous therapy, subcutaneous therapy, peg feeds or other appropriate treatments. To facilitate pain management, continence and bowel management as required. To plan and implement care that aims to prevent avoidable hospital admission. To optimise opportunity to provide care in patients preferred place, including where practicable, terminal care. Governance To accept responsibility as a professional for his/her actions in relation to own work and work delegated to others, acting at all times within the Nursing and Midwifery Council Code of Conduct. To maintain knowledge of current practice and promote evidenced based practice. To maintain professional competence and personal development by participation in mandatory and appropriate training. To be responsible for maintenance of timely and accurate written and electronic patient records in line with professional and Your Healthcare policy ensuring confidentiality is maintained in accordance with the Data Protection Act and DOH policy on patient consent. To demonstrate understanding of clinical governance and risk management and integrate this into practice. To participate in audit and standard setting. To plan and participate in the induction of new staff. To participate in a supervision and appraisal process and ensure the development and maintenance of a personal development plan which contributes to team and personal objectives. To have an awareness and adhere to Joint Safeguarding of Vulnerable Adults Policies and Safeguarding Children Policies. To facilitate a learning environment for pre and post registration nursing students and students from other health and social care settings. To participate in and ensure team compliance in the collection of statistical information which informs performance data for the team. To promote integrated team working and adherence to YHC policies. To effectively communicate relevant local and national initatives to the team. To practice at all times within a clinical governance framework that promotes quality. Management To ensure the smooth and efficient running the Community Nursing Team in partnership with the Locality Manager, further developing the service to meet the needs of the population. To provide line management for the team of qualified and unqualified nursing staff including appropriate delegation and organisation of workload, management of absence and performance and ensuring adequate staffing. To undertake supervision and appraisals of team members in order to develop personal development plans, contribute to team and personal objectives and ensure ongoing assessment of staff performance and conduct. To participate in recruitment and induction of new staff. To participate in the development and integration of care pathways, policies and procedures, which will influence service delivery and practice. To support an environment that is patient focused and enables the nursing team to challenge all aspects of patient care. To demonstrate excellent communication skills in the management of conflict across a range of circumstances. To be a role model through the exercise of good personal time management, punctuality, consistent reliable attendance and high quality care provision. To deputise for the Locality Team Manager as appropriate. Communication and Working Relationships. To provide strong leadership and promote team working. To establish robust and effective communication networks and working relationships with the wider district nursing service, through attendance at team meetings and regular caseload meetings. To ensure effective communication between the team and clients/families and other stakeholders in both statutory and voluntary sectors. To evaluate, adopt or adapt communication methods as appropriate to educational/social background and recognising barriers to this communication. To initiate and participate in team meetings and facilitate/attend organisational meetings or working parties as required/delegated by the Locality Team Manager. To ensure all information from meetings is documented and disseminated to all team members and that recommendations are implemented as necessary. To develop and maintain communication links with ward staff, promoting the importance of community team involvement in discharge planning.
