There is a mix of office and home working in this role. No two days are ever the same in the Adults and Autism Team, working with a wide range of individuals and situations. The team is supportive and knowledgeable, with people working together in a flexible way for the best outcomes for those we work with. Learning Disability Partnership Huntingdonshire Team: 21 to30 hours based at Scott House in Huntingdon. There is a mix of office and home working in this role. Learning Disability Partnership SouthTeam: 18.5hours based at SunleyHouse in Papworth. There is a mix of office and home working in this role. You will be undertaking Social Care and Mental Capacity Act assessments, completing Safeguarding as well as care and support planning, to achieve better outcomes for the adults we support.Inquisitive and creative practice is embraced, to allow for personalised solutions, taking into account wider community resources and organisations. South Transfer of Care Team: 22.5 hours basedpredominantly fromCambridge University Hospital (Addenbrookes) We provide support to adults being discharged from hospitals and rehabilitation setting. You will be imaginative and dynamic and make a positive contribution to delivering good outcomes for people who have care and support needs. You will be a person-centred practitioner with excellent communication skills and to be able work within a multidisciplinary setting.
